β LACRIMAL PROBING
π What is Lacrimal Probing?
Lacrimal probing is a procedure used to assess the puncta, canaliculi, and lacrimal drainage pathway and, in selected cases, to mechanically open a stenotic or obstructed segment.
π― INDICATIONS
β
Punctal stenosis
β
Canalicular stenosis or obstruction
β
Congenital nasolacrimal duct obstruction (CNLDO) when symptoms persist despite conservative management
β
Persistent epiphora when obstruction is suspected
β
Assessment/localization of a mechanical obstruction
π¬ BASIC PROCEDURE
1οΈβ£ Punctal dilation
The punctum is gently dilated.
2οΈβ£ Probe insertion
A lacrimal probe is introduced through the punctum and canaliculus.
3οΈβ£ Assessment
The pathway is assessed for stenosis, obstruction, or a hard stop.
4οΈβ£ Irrigation
Syringing may be performed to assess drainage patency.
πΆ IN CONGENITAL NLDO
The common problem is failure of canalization at the valve of Hasner.
If persistent, probing may be considered according to the child's age, symptoms, and ophthalmologist's assessment.
β οΈ IMPORTANT
Lacrimal probing is an invasive procedure and should be performed by a trained eye-care professional using appropriate sterile techniques.
π CLINICAL PEARL
Probing helps assess the anatomy of the lacrimal drainage pathway and may relieve certain mechanical obstructions.
