Today We did Thoracic Endo Vascular Aortic repair (TEVAR) on a 49yr old patient who presented with Acute Type B Aortic Dissection with our Cardiothoracic surgeon and interventional cardiologists. Prior to Aortic Stenting, in the operating room, we did left Neck dissection to Expose the subclavian Artery by dividing the Scalene anterior muscle.Then we Did Left carotid exposure after opening the carotid sheath to separate the IJV and Vagus from the Carotid. Left Carotid to Left Subclavian Bypass using a Dacron graft was done by CTVS surgeon. This was to prevent Endoleak and also to keep Perfusing Subclavian artery after stent deployment. Then In cath lab stent was placed. It was good team experience to give an optimal result to the patient.
This was followed by TEVAR in Cathlab lab. Procedure was successfully done without any complications.

Carotid Body tumor (Right) Shamblin type-III
Tumor excised without pre-op embolization
Blood loss 200ml
Duration 2.5hours
Vagus nerve engulfed by the tumor. Isolated and preserved using Medtronics NIM monitoring.
Post Op uneventful. Patient discharged on day-2
