▫️How to measure the 'Q-angle' Measurement.

▪️apparatus required~ long arm Goniometer.

▪️suitable position ~ standing & supine.

▪️procedure ~ Place the fulcrum of the goniometer over the centre of the patella and position the bottom arm in line with the patella tendon and tibial tuberosity. Next position the upper arm so that it is pointed directly at the anterior inferior iliac spine (AIIS) of the ilium (point to which re**us Femoris attaches).
The small angle of the goniometer is the Q angle.

39 :- Duchenne muscular dystrophy.

Duchenne muscular dystrophy is an inherited disorder. It involves muscle weakness, which quickly gets worse.

~CAUSES
1..Duchenne muscular dystrophy is a form of muscular dystrophy. It worsens quickly. Other muscular dystrophies (including Becker muscular dystrophy) get worse much more slowly.

2..Duchenne muscular dystrophy is caused by a defective gene for dystrophin (a protein in the muscles). However, it often occurs in people without a known family history of the condition.

3..The condition most often affects boys due to the way the disease is inherited. The sons of women who are carriers of the disease (women with a defective gene, but no symptoms themselves) each have a 50% chance of having the disease. The daughters each have a 50% chance of being carriers. Very rarely, a female can be affected by the disease.

4..Duchenne muscular dystrophy occurs in about 1 out of every 3,600 male infants. Because this is an inherited disorder, risks include a family history of Duchenne muscular dystrophy.

SYMPTOMS
Symptoms most often appear before age 6.

Symptoms may include:

1.Fatigue
2.Learning difficulties (the IQ can be below 75)
3.Intellectual disability (possible, but does not get worse over
time)
4.Muscle weakness:

5.Begins in the legs and pelvis, but also occurs less severely
in the arms, neck, and other areas of the body
6.Problems with motor skills (running, hopping, jumping)
7.Frequent falls
8.Trouble getting up from a lying position or climbing stairs
9.Shortness of breath, fatigue and swelling of the feet due to a
weakening of the heart muscle
10.Problem breathing due to a weakening of the respiratory
muscles
11.Gradual worsening of muscle weakness
12.Progressive difficulty walking:
13.Ability to walk may be lost by age 12, and the child will
have to use a wheelchair.
14.Breathing difficulties and heart disease often start by age .

EXAMINATION & TESTS
A complete nervous system (neurological), heart, lung, and muscle exam may show:

-Abnormal heart muscle (cardiomyopathy) becomes evident by the age of 10.
-Congestive heart failure or irregular heart rhythm (arrhythmia) is present in all people with Duchenne muscular dystrophy by the age of 18.
-Deformities of the chest and back (scoliosis).
-Enlarged muscles of the calves, buttocks, and shoulders (around age 4 or 5). These muscles are eventually replaced by fat and connective tissue (pseudohypertrophy).
-Loss of muscle mass (wasting).
-Muscle contractures in the heels, legs.
-Muscle deformities.
-Respiratory disorders, including pneumonia and swallowing with food or fluid passing into the lungs (in late stages of the disease).

TESTS MAY INCLUDE:

Electromyography (EMG)
Genetic tests
Muscle biopsy
Serum CPK

TREATMENT :-
-There is no known cure for Duchenne muscular dystrophy. -Treatment aims to control symptoms to improve quality of life.

-Steroid drugs can slow the loss of muscle strength. They may be started when the child is diagnosed or when muscle strength begins to decline.

Other treatments may include:

-Albuterol, a drug used for people with asthma
-Amino acids
-Carnitine
-Coenzyme Q10
-Creatine
-Fish oil
-Green tea extracts
-vitamin E

The use of steroids and the lack of physical activity can lead to excessive weight gain. Activity is encouraged. Inactivity (such as bedrest) can make the muscle disease worse. Physical therapy may help to maintain muscle strength and function. Speech therapy is often needed.

Other treatments may include:

Assisted ventilation (used during the day or night)
Drugs to help heart function, such as angiotensin converting enzyme inhibitors, beta blockers, and diuretics
Orthopedic appliances (such as braces and wheelchairs) to improve mobility
Spine surgery to treat progressive scoliosis for some people
Proton pump inhibitors (for people with gastroesophageal reflux)

The sympathetic nervous system controls the body responses to perceived threat and is responsible for the 'fight or flight' response.

36:- GLIOBLASTOMA MULTIFORME
Glioblastomas (GBM) are tumors that arise from astrocytes—the star-shaped cells that make up the “glue-like,” or supportive tissue of the brain. These tumors are usually highly malignant (cancerous) because the cells reproduce quickly and they are supported by a large network of blood vessels.
:-LOCATION
Glioblastomas are generally found in the cerebral hemispheres of the brain, but can be found anywhere in the brain or spinal cord.

:-SYMPTOMS
~increased pressure in the brain.
~headache
~nausea
~vomiting
~drowsiness.
Depending on the location of the tumor, patients can develop a variety of other symptoms such as weakness on one side of the body, memory and/or speech difficulties, and visual changes.

:-TREATMENT
Glioblastoma can be difficult to treat because the tumors contain so many different types of cells. Some cells may respond well to certain therapies, while others may not be affected at all. This is why the treatment plan for glioblastoma may combine several approaches.

The first step in treating glioblastoma is a procedure to make a diagnosis, relieve pressure on the brain, and safely remove as much tumor as possible through surgery. Because glioblastomas have finger-like tentacles, they are very difficult to completely remove. This is particularly true when they are growing near the parts of the brain that control important functions such as language and coordination.

Radiation and chemotherapy may be used to slow the growth of tumors that cannot be removed with surgery. Chemotherapy may also be used to delay the need for radiation in young Children.

35 :- Hemophilia
"Hemophilia" is a bleeding disorder inherited as a sex-linked autosomal recessive trait (males are affected, females are carriers). The condition is caused by an abnormality of plasma clotting proteins (clotting factor VIII in hemophilia A; clotting factor IX in hemophilia B or Christmas disease) necessary for blood coagulation, producing a prolonged but not faster bleeding than would occur in a normal person with the same injury.

★Two primary types:-
=>Hemophilia A (classic hemophilia-factor VIII deficiency 80% of all cases.
=>Hemophilia B. (Christmas disease-factor IX deficiency).

★Clinical Features of Hemophilia:-
=>The hallmark of hemophilia is hemorrhage into the joints. This bleeding is painful and leads to long-term inflammation and deterioration of the joint, resulting in permanent deformities, misalignment, loss of mobility, and extremities of unequal lengths.

★Complications with Hemophilia
Complications include:
●Joint contracture(s) and deformities (especially at hip, knee, elbow, and ankle joint").
●Hemophiliac arthropathy, which may occ forms: the articular cartilage softens, turns brown (due to hemosiderin), and becomes pitted and fragmented.
●Muscle weakness and atrophy around affected joints.
●Peripheral nerve compression by hematoma.
●postural scoliosis.
●Decreased aerobic fitness.
●Difficulties with ADLs.

★Physiotherapy role :-
◆PRICE protocol in case of acute Bleeding , It reduces swelling and tissue damage when used together with clotting factor concentrates.
◆Prevention of contractures:
=>Manual traction and mobilization.
=>Progressive/dynamic splinting.
=>Serial casting/drop-out casts.
=>Active ROM exercises (passive ROM is generally contraindicated).
◆Maintaining strength:
=>Isometric strengthening exercises initially, then graded progressive exercises.
=>Exercise in a pool, hot soaks in a tub, or whirlpool baths. It is easier to move the body in water. Good for aerobic conditioning.
◆Preventing or diminishing disability:
=> Gait training.
=> Proprioceptive training.
=> Bracing/splints -provides stabilization and protection.
