*Myth:* “Too much calcium = No stent” ❌
*Fact:* With Rota, even rock-hard arteries take stents perfectly ✅

2 cases from my cath lab this week:

Case 1 —Mid RCA 95% with heavy circumferential calcium.
Strategy: Rotational atherectomy → balloon predilation → DES implantation.

Result: Excellent stent expansion, TIMI 3 flow.

Case 2 —Proximal LAD 100% calcified blockage.
Strategy: Rotational atherectomy → high-pressure NC balloons → DES deployment.

Result: Optimal expansion, no malapposition, widely patent vessel.

Key Takeaway: Severe calcium is not a contraindication to stenting — but it is a contraindication to inadequate preparation.

Rota creates the compliant channel needed for perfect stent deployment.

Respect the calcium. Prepare the lesion. Deliver the result.

Heart disease is hitting younger patients hard — especially with diabetes, hypertension, or family history. Know your risk.

“Zero Metal Weekend”

Stent or no stent?

Not every Blockage needs STENT
In many cases, a drug-coated balloon (DCB) can treat blockage without needing stent and keeping the artery metal (Stent) free.

Three cases from my cath lab this week:*

Case 1 — 48M, smoker, high-stress job, irregular lifestyle. Presented with crushing chest pain. RCA 99% in-stent restenosis.
Result: DCB angioplasty only. No stent. Artery beautifully opened, excellent flow, pain resolved.

Case 2 — 34M, sedentary, strong family history, junk food, undiagnosed hypertension. Collapsed at home. LAD-D1 100% thrombotic occlusion.
Result: DCB only. No stent. Vessel successfully opened with great results.

Case 3 — Young patient, acute chest pain, multiple risk factors. LAD-D1 severe 95% lesion.
Result: DCB angioplasty only. No stent. Excellent angiographic outcome.

Three patients. Three critically blocked arteries. Zero metal implanted.

What these cases prove:
1. Heart disease doesn’t check your age. Smoking, stress, poor diet, inactivity, and hypertension are causing MIs in patients under 50.
2. Stent isn’t always the answer. For ISR, small vessels, bifurcations, and select de-novo lesions — meticulous prep + DCB delivers DES-level drugs without a permanent scaffold.

Key principle: No prep, no DCB. This is precision PCI, not a shortcut.

If you think “it can’t be my heart,” please reconsider.
Check your blood pressure. Move daily. Quit smoking. Know your family history.

MY WEEKEND IN THE CATH LAB!!!

*Saturday 2 AM:* 28-year-old gentleman. Smoker, high-stress lifestyle, irregular sleep, no regular exercise. Crushing chest pain. Right Coronary Artery 100% blocked. Stent placed. Pain gone.

*Sunday 4 PM:* 34-year-old gentleman . Sedentary work, strong family history of heart disease, frequent junk food, undiagnosed hypertension. Collapsed at home. Left Main Coronary Artery 100% blocked. Emergency angioplasty. Stable now.

Two young hearts, two lives saved. Both had modifiable risk factors.
