Dr Atul Shanker receiving Lifetime Achievement Award from the national president of Indian Academy of Pediatrics.

Some people know him as a pediatrician.
Thousands know him as the doctor who cared for their children.
I know him simply as my father.

My first teacher.
My greatest mentor.
The person who taught me that medicine is not just about treating illness, but about serving with compassion, humility, and integrity.

Watching him receive the Lifetime Achievement Award in Pediatrics is more than a proud moment—it is witnessing a lifetime of quiet sacrifices, countless sleepless nights, and generations of children whose lives are better because of him.

No award can truly measure a life dedicated to healing.
But this one reminds us that such lives deserve to be celebrated.

Congratulations, Dad. Your greatest legacy isn’t the award—it’s the thousands of lives you’ve touched, and the values you’ve passed on to me.

1. Gut microbiota means the billions of tiny bacteria living in our intestines that help keep us healthy.

2. A good gut has many different types of helpful bacteria, not just one or two kinds.

3. Beneficial bacteria like Bifidobacterium, and Lactobacillus help maintain gut health.

4. Healthy gut bacteria produce important substances called short-chain fatty acids (SCFAs) that nourish the cells of the intestine.

5. These substances reduce inflammation and strengthen the gut lining, preventing harmful substances from entering the body

6. A good gut microbiota protects against harmful germs by occupying space and producing natural antibacterial compounds

7. Gut bacteria help digest food and produce vitamins such as vitamin K and some B vitamins.

8. The gut microbiota supports the immune system, helping the body respond properly to infections.

9. Gut bacteria also communicate with the brain through the gut–brain connection, affecting mood and mental health.

10. When good bacteria decrease and harmful ones increase (called dysbiosis), it may contribute to digestive problems, inflammation, and other health issues.

A “good” gut microbiota refers to a balanced, resilient, and metabolically efficient microbial ecosystem in the gastrointestinal tract rather than just microbial diversity. Healthy microbiota show high diversity and stability with beneficial bacteria such as Bifidobacterium, Faecalibacterium prausnitzii, Akkermansia muciniphila, and some Lactobacillus species.

These microbes produce short-chain fatty acids (SCFAs)—especially butyrate, propionate, and acetate—which supply energy to colon cells, maintain intestinal barrier integrity, and regulate immune responses through regulatory T-cells (Treg). Butyrate also influences gene expression by regulating histone deacetylase (HDAC) activity.

A healthy microbiota supports mucosal barrier function, controls lipopolysaccharide exposure, and prevents pathogen growth through competitive exclusion and antimicrobial compounds. It also contributes to bile acid metabolism, vitamin synthesis (K and B-complex), and tryptophan metabolism, influencing systemic immunity and the gut–brain axis.

In contrast, dysbiosis—reduced diversity and harmful microbial overgrowth—is associated with metabolic, inflammatory, and neuropsychiatric disorders.

Sleep architecture is profoundly modulated by dietary composition, meal timing,
micronutrient density, and metabolic signaling. Food influences sleep through neurochemical,
endocrine, circadian, and inflammatory pathways.
Tryptophan-containing foods (e.g., dairy, nuts, legumes) increase central serotonin synthesis
via competitive transport across the blood–brain barrier, subsequently enhancing melatonin
production in the pineal gland. High–glycemic index carbohydrates accelerate insulin-
mediated uptake of competing large neutral amino acids into skeletal muscle, transiently
increasing plasma tryptophan ratio and potentially shortening sleep onset latency. Conversely,
excessive simple sugars may provoke nocturnal sympathetic activation and sleep
fragmentation.

A 200-word clinical insight

The first 1000 days of life — from conception to approximately 2 years of age — represent the most critical window for brain development. During this period, the brain undergoes rapid neurogenesis, synaptogenesis, myelination, and synaptic pruning. Nearly 80–90% of structural brain growth occurs before the third birthday. Neural circuits responsible for social communication, language acquisition, executive function, emotional regulation, and cognitive processing are being wired at extraordinary speed.

Optimal nutrition (including maternal health, breastfeeding, micronutrients like iron, iodine, DHA), responsive caregiving, secure attachment, sensory stimulation, and protection from toxic stress are foundational inputs that shape long-term IQ, social competence, and psychological resilience. Conversely, chronic stress, malnutrition, neglect, screen overexposure, or untreated medical conditions during this window can alter neural architecture and stress-regulation systems.

Early experiences literally sculpt the brain’s circuitry. Investment in the first 1000 days is not merely about survival — it is about optimizing neurological potential, mental health stability, learning capacity, and lifelong social functioning. Preventive care, developmental screening, and timely intervention during this period yield the highest neurodevelopmental return on investment across the lifespan.

National Module on Neurometabolic Conditions.
Participated as national faculty and expert reviewer of the module.

Neurometabolic conditions are disorders in which defects in metabolic pathways impair normal brain structure and function.

They commonly involve abnormalities in mitochondrial energy production, amino acid metabolism, organic acids, lysosomal function, or fatty acid oxidation.

Clinical manifestations often include developmental delay, seizures, regression, movement disorders, cognitive impairment, and psychiatric symptoms.

Many are genetic in origin (often autosomal recessive) and may present in infancy or childhood, though some manifest later in life.

Early diagnosis through metabolic screening and genetic testing is critical, as several neurometabolic conditions are treatable with diet, cofactors, enzyme replacement, or targeted therapies.

Growing up, I never imagined participating and contributing in a national pedicon with my father. From TOTs to session speaker, from learning to teaching - this whole experience was surreal at a personal level.
