An accessory cystic artery is an anatomical variation where an additional, smaller blood vessel supplies blood to the gallbladder in addition to the primary cystic artery.
These vascular variations occur in roughly 10% to 20% of people. They are highly relevant in hepatobiliary procedures, especially surgeries like a cholecystectomy (gallbladder removal), because unrecognized accessory vessels can result in unexpected, difficult-to-control intraoperative bleeding.

অ্যাক্সেসরি সিস্টিক আর্টারি (অতিরিক্ত সিস্টিক ধমনী) হলো এক ধরনের শারীরস্থানিক বৈচিত্র্য, যেখানে প্রধান সিস্টিক ধমনীর পাশাপাশি আরও একটি ছোট রক্তনালী পিত্তথলিতে রক্ত সরবরাহ করে।
​এই ধরনের রক্তনালীর বৈচিত্র্য প্রায় ১০% থেকে ২০% মানুষের মধ্যে দেখা যায়। হেপাটোবিলিয়ারি সার্জারির ক্ষেত্রে এগুলো অত্যন্ত গুরুত্বপূর্ণ, বিশেষ করে কোলেসিস্টেক্টমি (পিত্তথলি অপসারণ) এর মতো অস্ত্রোপচারের সময়। কারণ, অস্ত্রোপচারের সময় এই অতিরিক্ত রক্তনালীগুলো শনাক্ত না করা হলে তা অপ্রত্যাশিত এবং নিয়ন্ত্রণ করা কঠিন এমন রক্তপাতের সৃষ্টি করতে পারে।

When no one can understand the anatomy except the surgeon, then this is empyema gallbladder. The patient was operated on laparoscopically and uneventfully, though this was the toughest empyema I have ever seen. I should have taken initial pictures where all structures were adhered to the abdominal wall.

​"যখন সার্জন ছাড়া অ্যানাটমি (শারীরস্থান) আর কেউই বুঝতে পারে না, তখনই সেটা এম্পায়মা গলব্লাডার (Empyema Gallbladder)। অত্যন্ত জটিল ও কঠিন পরিস্থিতি থাকা সত্ত্বেও ল্যাপারোস্কোপিক্যালি এবং কোনো রকম অঘটন ছাড়াই রোগীর অপারেশন সম্পন্ন হয়েছে, যদিও এটি আমার দেখা এ যাবৎকালের সবচেয়ে কঠিন এম্পায়মা কেস ছিল। শুরুতে যখন সমস্ত স্ট্রাকচার পেটের দেয়ালের সাথে শক্তভাবে লেগে (adhered) ছিল, তখনকার কিছু ছবি তুলে রাখা উচিত ছিল।"

The anatomical plane of the middle hepatic vein runs directly to the gallbladder fossa. While usually situated safely deep within the liver, in roughly 10% to 20% of patients, large branches of this vein lie immediately adjacent to (or directly adherent to) the gallbladder bed.

মিডল হেপাটিক ভেইনের (middle hepatic vein) অ্যানাটমিক্যাল প্লেন বা শারীরবৃত্তীয় তলটি সরাসরি গলব্লাডার ফোসা (পিত্তথলির খাঁজ) পর্যন্ত বিস্তৃত থাকে। সাধারণত এটি যকৃতের (লিভার) বেশ গভীরে নিরাপদে অবস্থান করলেও, প্রায় ১০% থেকে ২০% রোগীর ক্ষেত্রে এই শিরার বড় শাখাগুলো সরাসরি গলব্লাডার বেড বা পিত্তথলির তলদেশের একেবারে সংলগ্ন (অথবা সরাসরি লেগে) থাকে।

Don't ignore your Gall bladder stone.An asymptomatic small gall bladder stone can Rotten your Gall bladder if it obstruct Cystic duct lumen.
In case of Diabetic patients sometimes pt don't feel any pain due to autonomic neuropathy. Pt operated Laparoscopically and discharged just 1 day after the surgery.
If surgery was not performed at right time this may lead to perforation and peritonitis and increase the chance of mortality and morbidity..

Definition: The cystic duct meets the common hepatic duct in its lower third, often closer to the duodenum (lower 10% of cases).

Surgical Risk: This configuration can cause the cystic duct to run parallel to the CHD (often for over 2 cm) or create a low medial insertion, which makes identifying the hepatocystic triangle difficult and increases the risk of cutting the CBD inadvertently.

Retrograde appendicectomy is used when the appendix is retrocaecal and adherent, making the tip difficult to access. In this technique, the surgeon starts at the base by dividing and ligating it first, then controls the appendiceal vessels, and finally delivers the appendix from base to tip. In contrast, normal appendicectomy begins at the tip and proceeds towards the base. This approach makes retrograde appendicectomy safer in difficult cases where the appendix is hidden or fixed.
