SITUS INVERSUS TOTALIS, a congenital anomaly causing mirror-image transposition of Abdominal & Thoracic viscerae. In other words, the organs which generally happen to be on the right side, are placed on left side & vice-versa, like Heart on Right, Liver & Gall bladder in left.

This is a rare anomaly arising in 1 in every 10,000 individuals.

All abdominal surgeries & their diagnosis becomes challenging in such a patient beacuse of rarity of the condition as well as most of the surgeons are used to do surgeries in a usual position. Performing Laparoscopic Cholecystectomy (Gall bladder removal surgery) in these patients is especially challenging because of difficult ergonomics.

I am thankful to God to operate in such a patient with Gall stone disease and the operation with post-op period being totally uneventful. Patient was discharged on 1st post-op day with a smiling face.

Another Completion Stump Cholecystectomy..

This time, A gentleman had Open Gall bladder removal (Subtotal) surgery done in 2024 & then presented with a large 15 mm stone in remnant part of Gall Bladder with immense abdominal pain. Diagnosed as Gall bladder stump stone and then planned for operation.

Intra-operatively, extensive adhesions were found and lysis been done meticulously. After an hour's effort, GB stump was identified after carefully dissecting Colon & Duodenum. Then the stump with a large stone was removed after standard CA & CD division.

The whole post-op period was uneventful needing no Blood transfusion or ICU care. Patient was discharged on POD-2.

Thanks to almighty and my assistants and full team of hospital for their cumulative efforts.

GALL STONES EVEN AFTER GALL BLADDER REMOVAL SURGERY ??

YES. Not a fault of your surgeon but an unusual and rare complication. It can happen after Open or Laparoscopic cholecystectomy.

A known but unusual complication after Gall bladder removal surgery, may happen both after Open or Lap Gall Bladder removal surgeries. This condition develops due to enlargement of the remnant part of Gall bladder or Cystic stump enlargement with formation of stone inside it. If associated with abdominal pain, then known as Stump Cholecystitis.

Management is removal of the remnant part of Gall bladder which is known as Stump Cholecystectomy, preferably by Laparoscopic approach.
