𝗔 𝗥𝗲𝗹𝗶𝗮𝗯𝗹𝗲 𝗔𝗰𝗰𝗲𝘀𝘀 𝗳𝗼𝗿 𝗛𝗲𝗺𝗼𝗱𝗶𝗮𝗹𝘆𝘀𝗶𝘀

For patients with Chronic Kidney Disease (CKD) who need regular hemodialysis, a tunnelled dialysis catheter can provide 𝗿𝗲𝗹𝗶𝗮𝗯𝗹𝗲 𝗮𝗰𝗰𝗲𝘀𝘀 𝘁𝗼 𝘁𝗵𝗲 𝗯𝗹𝗼𝗼𝗱𝘀𝘁𝗿𝗲𝗮𝗺 when a fistula is not yet available or cannot be created.

🔹 𝗪𝗵𝗮𝘁 𝗶𝘀 𝗮 𝗧𝘂𝗻𝗻𝗲𝗹𝗹𝗲𝗱 𝗗𝗶𝗮𝗹𝘆𝘀𝗶𝘀 𝗖𝗮𝘁𝗵𝗲𝘁𝗲𝗿?

It is a soft tube placed into a large vein, usually in the neck, and then 𝘁𝘂𝗻𝗻𝗲𝗹𝗹𝗲𝗱 𝘂𝗻𝗱𝗲𝗿 𝘁𝗵𝗲 𝘀𝗸𝗶𝗻 before coming out through the chest. It connects to the dialysis machine.

✅ 𝗞𝗲𝘆 𝗔𝗱𝘃𝗮𝗻𝘁𝗮𝗴𝗲𝘀

• 𝗖𝗮𝗻 𝗯𝗲 𝘂𝘀𝗲𝗱 𝗳𝗼𝗿 𝗹𝗼𝗻𝗴-𝘁𝗲𝗿𝗺 𝗱𝗶𝗮𝗹𝘆𝘀𝗶𝘀
• 𝗖𝗮𝗻 𝗯𝗲 𝘂𝘀𝗲𝗱 𝘀𝗼𝗼𝗻 𝗮𝗳𝘁𝗲𝗿 𝗶𝗻𝘀𝗲𝗿𝘁𝗶𝗼𝗻
• 𝗡𝗼 𝗿𝗲𝗽𝗲𝗮𝘁𝗲𝗱 𝗻𝗲𝗲𝗱𝗹𝗲 𝗽𝗿𝗶𝗰𝗸𝘀 during dialysis
• 𝗠𝗼𝗿𝗲 𝘀𝘁𝗮𝗯𝗹𝗲 𝗮𝗻𝗱 𝘀𝗲𝗰𝘂𝗿𝗲 than a temporary catheter
• 𝗟𝗼𝘄𝗲𝗿 𝗶𝗻𝗳𝗲𝗰𝘁𝗶𝗼𝗻 𝗿𝗶𝘀𝗸 than a non-tunnelled temporary catheter
• Useful when an 𝗔𝗩 𝗳𝗶𝘀𝘁𝘂𝗹𝗮 𝗰𝗮𝗻𝗻𝗼𝘁 𝗯𝗲 𝗰𝗿𝗲𝗮𝘁𝗲𝗱 or is not yet ready

⚠️ 𝗥𝗲𝗺𝗲𝗺𝗯𝗲𝗿

A tunnelled catheter can still develop 𝗶𝗻𝗳𝗲𝗰𝘁𝗶𝗼𝗻, 𝗯𝗹𝗼𝗰𝗸𝗮𝗴𝗲 or 𝗰𝗹𝗼𝘁𝘁𝗶𝗻𝗴.

For suitable patients, an 𝗔𝗩 𝗳𝗶𝘀𝘁𝘂𝗹𝗮 𝗼𝗿 𝗴𝗿𝗮𝗳𝘁 is generally preferred for long-term dialysis. The choice of dialysis access should always be individualized by your nephrologist.

Feeling unusually 𝗧𝗜𝗥𝗘𝗗, 𝗪𝗘𝗔𝗞 or 𝗕𝗥𝗘𝗔𝗧𝗛𝗟𝗘𝗦𝗦?

Anemia is very common in people with CKD — but 𝗶𝘁 𝗶𝘀 𝘁𝗿𝗲𝗮𝘁𝗮𝗯𝗹𝗲 𝘄𝗵𝗲𝗻 𝗱𝗲𝘁𝗲𝗰𝘁𝗲𝗱 𝗲𝗮𝗿𝗹𝘆.

🔴 𝗪𝗛𝗬 𝗗𝗢𝗘𝗦 𝗔𝗡𝗘𝗠𝗜𝗔 𝗢𝗖𝗖𝗨𝗥 𝗜𝗡 𝗖𝗞𝗗?

• Diseased kidneys produce less 𝗲𝗿𝘆𝘁𝗵𝗿𝗼𝗽𝗼𝗶𝗲𝘁𝗶𝗻 (EPO), the hormone needed to make red blood cells
• 𝗜𝗿𝗼𝗻 𝗱𝗲𝗳𝗶𝗰𝗶𝗲𝗻𝗰𝘆 or reduced availability of iron
• Chronic inflammation
• Blood loss, including during dialysis

Regular blood tests are important:

✅ 𝗛𝗲𝗺𝗼𝗴𝗹𝗼𝗯𝗶𝗻 (𝗛𝗯)
✅ Complete blood count
✅ 𝗙𝗲𝗿𝗿𝗶𝘁𝗶𝗻 & 𝗧𝗦𝗔𝗧 — to assess iron status
✅ Vitamin B12 & folate when indicated
✅ Evaluation for blood loss, inflammation and other causes

💊 𝗛𝗢𝗪 𝗜𝗦 𝗔𝗡𝗘𝗠𝗜𝗔 𝗧𝗥𝗘𝗔𝗧𝗘𝗗?

Treatment depends on the cause and the individual patient.

🟢 𝗜𝗿𝗼𝗻 𝗿𝗲𝗽𝗹𝗮𝗰𝗲𝗺𝗲𝗻𝘁 — oral or intravenous iron when indicated

🟢 𝗘𝗦𝗔 𝘁𝗵𝗲𝗿𝗮𝗽𝘆 — erythropoiesis-stimulating agents may be used in selected patients

🟢 Correct vitamin B12 or folate deficiency

🟢 Treat ongoing blood loss and inflammation

🟢 Newer treatments such as 𝗛𝗜𝗙-𝗣𝗛 𝗶𝗻𝗵𝗶𝗯𝗶𝘁𝗼𝗿𝘀 may be considered in selected patients

⚠️ Treatment must be 𝗶𝗻𝗱𝗶𝘃𝗶𝗱𝘂𝗮𝗹𝗶𝘇𝗲𝗱. Hb should not be pushed excessively high during ESA treatment because this can increase cardiovascular and stroke risk.

🛡️ 𝗛𝗢𝗪 𝗖𝗔𝗡 𝗬𝗢𝗨 𝗣𝗥𝗘𝗩𝗘𝗡𝗧 𝗢𝗥 𝗠𝗔𝗡𝗔𝗚𝗘 𝗔𝗡𝗘𝗠𝗜𝗔?

✔️ Attend regular CKD follow-ups
✔️ Monitor Hb and iron status as advised
✔️ Take prescribed iron/vitamin supplements correctly
✔️ Maintain adequate nutrition appropriate for your CKD stage
✔️ Report persistent fatigue, breathlessness or dizziness
✔️ Identify and treat sources of blood loss

⚠️ 𝗣𝗢𝗦𝗦𝗜𝗕𝗟𝗘 𝗦𝗜𝗗𝗘 𝗘𝗙𝗙𝗘𝗖𝗧𝗦 𝗢𝗙 𝗧𝗥𝗘𝗔𝗧𝗠𝗘𝗡𝗧

• Iron tablets → stomach upset, constipation or dark stools
• IV iron → allergic/infusion reactions in some patients
• ESA therapy → increased blood pressure and, particularly with excessive Hb targets, increased risk of stroke and cardiovascular complications

💡 𝗞𝗘𝗬 𝗠𝗘𝗦𝗦𝗔𝗚𝗘

𝗔𝗻𝗲𝗺𝗶𝗮 𝗶𝗻 𝗖𝗞𝗗 𝘀𝗵𝗼𝘂𝗹𝗱 𝗻𝗼𝘁 𝗯𝗲 𝘀𝗶𝗺𝗽𝗹𝘆 𝗮𝗰𝗰𝗲𝗽𝘁𝗲𝗱 𝗮𝘀 “𝗽𝗮𝗿𝘁 𝗼𝗳 𝗸𝗶𝗱𝗻𝗲𝘆 𝗱𝗶𝘀𝗲𝗮𝘀𝗲.”

Early diagnosis, correction of iron deficiency and other reversible causes, and individualized treatment can improve symptoms and quality of life.
