Gallbladder with Phrygian cap deformity can occasionally pose a diagnostic challenge on ultrasonography. Small calculi lodged within the folded fundal segment may be overlooked if the Phrygian cap is not carefully evaluated. Therefore, thorough assessment of the gallbladder from fundus to neck in multiple scanning planes, along with correlation with clinical history and patient symptoms, is essential. Careful targeted evaluation is important because both fundal calculi within a Phrygian cap and impacted stones at the gallbladder neck may be missed on routine examination, potentially leading to false-negative diagnosis and persistent patient symptoms
