4 മില്ലീമീറ്ററിലേക്ക് ചുരുങ്ങിയ ശ്വാസനാളം:40-കാരന് ഇന്റർവെൻഷണൽ പൾമണോളജിയിലൂടെ ആശ്വാസം

റോഡപകടത്തെ തുടർന്ന് തലയ്ക്ക് ഗുരുതരമായി പരിക്കേൽക്കുകയും തലച്ചോറിൽ രക്തസ്രാവം (Intracranial Bleed) ഉണ്ടാവുകയും ചെയ്ത 40 വയസ്സുള്ള ഒരു രോഗി, രണ്ടാഴ്ചയോളം മറ്റൊരു ആശുപത്രിയിൽ വെന്റിലേറ്റർ സഹായത്തോടെയാണ് ജീവൻ നിലനിർത്തിയത്.

എന്നാൽ പിന്നീട് അദ്ദേഹത്തിന് കടുത്ത ശ്വാസതടസ്സം അനുഭവപ്പെടുകയും ശ്വാസമെടുക്കാൻ ബുദ്ധിമുട്ടുന്ന ഗുരുതരാവസ്ഥയിലേക്ക് (Type 2 Respiratory Failure) എത്തുകയും ചെയ്തു. വിശദമായ പരിശോധനയിൽ വെന്റിലേറ്റർ ട്യൂബ് ദീർഘനാൾ വെച്ചതുമൂലം ശ്വാസനാളം (Trachea) ചുരുങ്ങി വെറും 4 മില്ലീമീറ്റർ മാത്രമായി മാറിയ 'ട്രക്കിയൽ സ്റ്റെനോസിസ്' (Tracheal Stenosis) എന്ന സങ്കീർണ്ണാവസ്ഥയാണെന്ന് കണ്ടെത്തി.

തുടർന്ന് റിജിഡ് ബ്രോങ്കോസ്കോപ്പി (Rigid Bronchoscopy) വഴി സങ്കീർണ്ണമായ ശ്വാസനാളം വികസിപ്പിക്കുകയും അതിൽ സ്റ്റെന്റ് (Airway Stent) നിക്ഷേപിക്കുകയും ചെയ്തു. ഇതിലൂടെ ശ്വാസനാളത്തിന്റെ തടസ്സം പൂർണ്ണമായി മാറ്റി ശ്വാസോച്ഛ്വാസം സാധാരണ നിലയിലാക്കാൻ സാധിച്ചു.

കഴിഞ്ഞ രണ്ട് മാസമായി 72 വയസ്സുള്ള ഒരു അമ്മ വിട്ടുമാറാത്ത ശ്വാസകോശ ബുദ്ധിമുട്ടുകൾ കാരണം കഷ്ടപ്പെടുകയായിരുന്നു.

നെഞ്ചിന്റെ സിടി സ്കാൻ (CT Thorax) നോർമൽ ആയിരുന്നിട്ടും, രോഗലക്ഷണങ്ങൾ കൃത്യമായി മനസ്സിലാക്കി അവരെ എന്റെ അടുത്തേക്ക് റഫർ ചെയ്ത ഡോ.Caroline SP യുടെ മികച്ച രോഗനിർണ്ണയ പാടവം കൊണ്ടാണ് ഈ ബുദ്ധിമുട്ടിന്റെ യഥാർത്ഥ കാരണം കണ്ടെത്താൻ സാധിച്ചത്. ആഴ്ചകൾക്ക് മുൻപ് അബദ്ധത്തിൽ ശ്വാസകോശത്തിന്റെ ഉള്ളിൽ പോയ ഒരു 'കടല' (Bengal gram) ആയിരുന്നു വില്ലൻ.
