'Gun-Barrel' approach to 'Percutaneous Balloon Kyphoplasty', thanks to our own and Legendary Pain Physician, Neeraj Jain Sir..

OVCF of L5 but with H/O Ca-Prostate as well, severely painful and ADL limiting, and at Pain Medicine Unit of Rmlims Lucknow, we used a modified approach for doing unipedicular approach to reach contralateral side, we also employed a 'diffrential Balloon tamp inflation' to achievea better creation. and got a good fill with 13 ml cement and 'pedicle to pedicle' and a painfree patient, walking comfortably after half an hour..

Patients with chronic pain syndromes often push the boundaries of clinical knowledge, skill, and endurance of every Pain Physician.

In such moments, it is the unwavering faith in science—combined with a quiet surrender to the divine—that keeps us moving forward.

Today’s case was one of those profound tests.

A patient with carcinoma mandible—without mandible, maxilla, or supporting musculature. No bony landmarks. Extremely poor fluoroscopic visibility. Severe, unrelenting facial pain, unresponsive even to narcotics.

The Pain Medicine team at Dr. RMLIMS, Lucknow took a leap of faith—guided by experience, anatomy, and perseverance—to identify the elusive foramen ovale and reach the Gasserian ganglion for intervention.

With absent musculature, motor stimulation was not feasible. Yet, upon sensory stimulation, concordant paraesthesia was achieved—guiding the path forward.

Radiofrequency was performed.

And in the postoperative period—the reward: markedly reduced pain.

Moments like these remind us why Pain Medicine is not just a specialty, but a calling.

With deep gratitude, I bow my head to Bhagwan Almighty for allowing me to be a part of this journey.

Many patients challenge the conventional understanding of disease processes… and today’s patient was one such example.

A 28-year-old young male presented with moderate to severe back pain while sitting and walking, following a fall from a truck nearly 1.5 years ago. Examination revealed marked point tenderness over the lower dorsal–upper lumbar midline spine.

MRI showed:
• Vertebral compression fractures at D12 and L1
• Persistent bone marrow edema on STIR images
• A moderate-sized defect in the superior endplate of D12 (?Schmorl’s node), significantly increasing the risk of cement leakage during intervention.

And the icing on the cake — the patient is getting married in a week and desperately wanted to become pain-free… without any incision or stitches.

The Pain Medicine Team at Dr RMLIMS, Lucknow, India, offered one of the most advanced MIPSIs in Pain Medicine — *Percutaneous Balloon Kyphoplasty* of D12 and L1.

Despite the guarded prognosis considering the chronicity of injury, the procedure was performed using meticulous technique during kyphoplasty, with:
✔ No cement leakage into the disc
✔ Uneventful procedure
✔ Completely pain-free patient within one hour

The Right Pediculoplasty was unintended-)

Cases like these remind us that medicine is not merely about protocols and algorithms — it is also about clinical judgement, courage, experience, communication, and compassion.

As physicians, we should never hesitate to take a thoughtful “leap of faith” when we genuinely believe an intervention may help the patient — provided everything, especially potential complications, is transparently explained beforehand.

And sometimes, beyond science and skill, one must also believe in the healing grace of Almighty Bhagwan.

and Pain Medicine Unit of Dr RMLIMS Lucknow employed advanced MIPSI solution ie Percutaneous Balloon Kyphoplasty with 'Egg-Shelling Technique' to reduce the risk of cement leakage..

Result- reasonably good fill of both halves of collapsed vertebra with great amount of relief and walking once again patient.

No patient of vertebral collapse should be left with so-called 'conservative care' and miserable life, when such wonderful Minimally Invasive Pain and Spine Interventions are available..

Today's MIPSI involved a Balloon Kyphoplasty.

The patient is a 67-year-old female who has been bedridden for three months due to severe backache. An MRI revealed a new L2 osteoporotic vertebral compression fracture with edema, alongside a significantly displaced posterior fragment. A CT scan also indicated a broken pedicle on the left side, also very poor fluoroscopic visibility due to severe Osteoporosis (T-Score, -5.4), making it more complicated and difficult.

Despite these challenges, on the auspicious occasion of Sri Ram Navmi, and with divine grace, the Pain Medicine Unit at Dr. RMLIMS Lucknow successfully performed the procedure. We were able to place a balloon tamp via a right unipedicular approach, crossing the midline, and reconstruct the vertebra using a substantial 14 ml of PMMA.

The outcome was remarkable: the patient was able to walk pain-free within half an hour, and her anti-osteoporotic treatment has also commenced.

This was a deeply gratifying experience, and we sincerely hope that all patients suffering from osteoporotic vertebral compression fractures receive timely care, as it can truly transform their lives.
