Operated a very rare ectopic insulin-secreting NET involving the re**um and mesore**um—an exceptionally rare presentation, with only one similar case previously reported in the literature.
The patient presented with recurrent episodes of severe hypoglycaemia and was managed preoperatively with octreotide under the expert care of Dr. Vibhuti and the Department of Endocrinology.
A true example of multidisciplinary teamwork between Endocrinology and Surgical Gastroenterology, allowing successful optimization and definitive surgical management of this challenging case.
Imaging: A mass was identified in the mesore**um with multiple extra-tumoral deposits.
Considering the location and extent of disease, the patient underwent low anterior resection. Intraoperatively, the mass was found to extend up to the iliac artery, with breach of the presacral fascia.
The patient is doing well in the postoperative period.

For cancers of the lower re**um, preserving the a**l sphincter is always a desirable goal—but never at the expense of oncological safety.
Advances such as total neoadjuvant therapy (TNT), intersphincteric resection (ISR), and transa**l techniques have increased the possibility of sphincter preservation in carefully selected patients. However, these approaches are not appropriate for every patient. When the tumor involves the sphincter complex or an oncologically safe resection cannot be achieved, laparoscopic APR remains the optimal treatment, ensuring complete tumor removal while maintaining the principles of total mesore**al excision.

Extrahepatic Portal Venous Obstruction (EHPVO)
A serious but often under-recognized condition, EHPVO mainly
affects children and young adults.
🔬 What happens?
•The portal vein gets blocked and is replaced by multiple abnormal
venous channels
•This leads to reduced blood flow to the liver and portal
hypertension
🚨 Common
problems patients may face:
• Hematemesis(vomiting blood) due to variceal bleeding
• Massive splenomegaly (enlarged spleen)
• Lowplatelet count → increased risk of bleeding & infections
• Jaundice and gallstone disease
• Growth retardation in children, affecting overall development and schooling
🛠️ How can it be treated?
The patient usually requires medical and surgical management according to the condition.Many Patients benefit from surgical management, which can significantly improve the quality of life:
• Splenectomy –removal of the enlarged spleen
• Proximalsplenorenal shunt (PSRS) – connecting the splenic vein to the renal vein to decompress the portal system

I am pleased to share that our narrative review, “Gene, genetics and genetic medicines in gastroenterology: Current status and its future”, has been published in the prestigious World Journal of Gastroenterology.

In this review, we discuss the expanding role of genetics, genetic medicine, and targeted therapies in gastroenterology, highlighting how these advances are transforming the understanding, diagnosis, and management of various gastrointestinal disorders. We focus on how precision medicine approaches can contribute to improved clinical outcomes across a wide spectrum of GI diseases.
We hope this article will help better understand the genetic basis of gastrointestinal disordersand encourage the integration of genetic and targeted therapies into routine clinical practice.
