Operated a very rare ectopic insulin-secreting NET involving the re**um and mesore**um—an exceptionally rare presentation, with only one similar case previously reported in the literature.
The patient presented with recurrent episodes of severe hypoglycaemia and was managed preoperatively with octreotide under the expert care of Dr. Vibhuti and the Department of Endocrinology.
A true example of multidisciplinary teamwork between Endocrinology and Surgical Gastroenterology, allowing successful optimization and definitive surgical management of this challenging case.
Imaging: A mass was identified in the mesore**um with multiple extra-tumoral deposits.
Considering the location and extent of disease, the patient underwent low anterior resection. Intraoperatively, the mass was found to extend up to the iliac artery, with breach of the presacral fascia.
The patient is doing well in the postoperative period.
