Not every white patch is vitiligo — here’s how we tell them apart 🔍

The clinical clues that point to vitiligo:
✅ Sharply demarcated borders (not fuzzy or blending into normal skin)
✅ Complete depigmentation- milky white, not just lighter
✅ Leukotrichia (white hair) within the patch
✅ Woods lamp exam — enhanced, bright blue-white fluorescence with accentuated borders
✅ Biopsy — absence of melanocytes (confirmatory in doubtful cases)

Other hypopigmented conditions typically show incomplete pigment loss, ill-defined borders, no Woods lamp accentuation, and preserved melanocytes on histology.

Correct diagnosis = correct treatment plan. Never assume — confirm. 🩺

All white spots are NOT vitiligo — especially in children.

As parents, it’s natural to panic the moment you spot a white patch on your child’s skin. But not every hypopigmented patch is vitiligo. Common look-alikes in kids include:

🔸 Pityriasis versicolor – fungal, often on the trunk, with fine scaling
🔸 Pityriasis alba – dry, faintly scaly patches, common on the face after sun exposure
🔸 Sun spots– tiny white spots on sun-exposed skin
🔸 Nevus anemicus – present since birth- a little lighter than skin.
🔸 Vitiligo – well-defined, chalky-white, progressive patches

Vitiligo is an autoimmune condition where your immune system mistakenly attacks melanocytes — the cells that give your skin its colour. The result: white patches that can appear anywhere on the body.

It is NOT contagious. It is NOT caused by diet. And it is NOT your fault.

Signs to watch for:
🔸 Milky-white patches, often symmetric
🔸 Common sites: face, hands, feet, around the mouth and eyes
🔸 Premature greying of hair, eyebrows, or eyelashes
🔸 Patches that slowly enlarge over months to years
🔸 Sometimes triggered or worsened by stress, sunburn, or skin injury

Treatment options that actually work:

Not every birthmark needs urgent action, but knowing which ones do can save you months of worry.

🔸 Hemangiomas (raised, red “strawberry” marks) — these can grow rapidly in the first few months. Early topical medication can control growth and prevent complications. Don’t wait — early intervention matters most in the first year.

🔸 Pigmented birthmarks (can be lighter or darker than skin tone) — mostly harmless and often fade or stay stable through childhood. If treatment is desired for cosmetic reasons, laser is the gold standard — but we wait until 18+ years, once skin has fully matured.

📈 ESR — screens for underlying inflammatory activity

Will neem kadha actually cure your urticaria? Let’s fact-check this

If you’ve dealt with hives, you’ve probably been told to try neem kadha, haldi-nimbu paste, or a cold compress. But do these home remedies actually work?

Reality check: Chronic urticaria is a mast cell disorder involving the IgE pathway. Home remedies might offer temporary relief from the itching or inflammation, but they don’t address the underlying trigger.

⚠️ The real risk: Patients who rely on home remedies for weeks or months often see their urticaria/wheals worsen, while proper antihistamine treatment gets delayed — something that’s completely avoidable.

If you’ve had wheals for more than 6 weeks, this could be chronic urticaria — and it’s time to see a dermatologist instead of relying on home remedies.
