β€œCan’t Miss ECGs”
1. STEMI 🚨
ST elevation in contiguous leads β†’ acute coronary occlusion
πŸ‘‰ Act fast: PCI / thrombolysis

2. NSTEMI ⚠️
ST depression / T inversion + ↑ troponin
πŸ‘‰ High risk ACS β†’ early invasive strategy

3. Atrial Fibrillation 🎯
Irregularly irregular rhythm, no P waves
πŸ‘‰ Stroke risk β†’ anticoagulation

4. Atrial Flutter πŸ”
Sawtooth waves (II, III, aVF)
πŸ‘‰ Often rapid, may need cardioversion/ablation

5. SVT (AVNRT/AVRT) ⚑
Narrow complex tachycardia (150–250 bpm)
πŸ‘‰ Vagal β†’ Adenosine

6. Monomorphic VT ⚑
Wide complex tachycardia
πŸ‘‰ Treat as VT unless proven otherwise

7. Ventricular Fibrillation πŸ’€
Chaotic rhythm, no pulse
πŸ‘‰ Immediate defibrillation = life-saving

8. Long QT Syndrome ⏳
Prolonged QT β†’ risk of torsades
πŸ‘‰ Avoid QT drugs, MgSOβ‚„ if unstable

9. 3rd Degree AV Block πŸ’”
AV dissociation, slow escape rhythm
πŸ‘‰ Needs pacing

10. Mobitz I (Wenckebach)
Progressive PR prolongation β†’ dropped beat
πŸ‘‰ Usually benign

11. Mobitz II ⚠️
Fixed PR with dropped QRS
πŸ‘‰ Dangerous β†’ pacemaker

12. Pericarditis πŸ”₯
Diffuse ST elevation + PR depression
πŸ‘‰ Pain better on leaning forward

β€œIn emergencies, ECG recognition is not knowledgeβ€”it’s survival.”

The "Scab-Like Sign" in Chronic Cavitary Aspergillosis

This is an excellent imaging finding! The post highlights an important and distinctive radiological sign in chronic cavitary pulmonary aspergillosis (CCPA).
What is the "Scab-Like Sign"?
The scab-like sign (also called the "nodular sign" or "granular appearance") refers to:
Multiple small nodular densities within or at the margins of a cavity
A granular, irregular texture resembling a scab on the cavity wall
Represents fungal colonization and inflammatory material within the cavity
Appears as a crinkled or wrinkled appearance of the cavity lining on HRCT
Radiological Characteristics (from the images shown)
Images a, b, & c demonstrate:
Multiple cavities with irregular walls (particularly bilateral upper lobe involvement)
Nodular/granular density at cavity margins β€” the "scab-like" appearance
Extensive bronchiectatic changes surrounding the cavities
Fibrotic stranding in the surrounding lung parenchyma
Traction bronchiectasis (bronchial dilatation from fibrosis)
Clinical Significance
Feature
Significance
Scab-like appearance
Indicates active fungal colonization and inflammation
Multiple cavities
Suggests progressive disease; often bilateral
Irregular cavity walls
Differentiates from TB (which has smooth walls)
Associated bronchiectasis
Common in CCPA; suggests chronic airway damage
Fibrotic changes
Indicates chronicity and tissue remodeling
Differential Diagnosis
The scab-like sign helps differentiate CCPA from:
Tuberculosis β€” TB cavities typically have smooth walls and are often solitary
Fungal ball (aspergilloma) β€” appears as a mobile, dense mass within cavity; NOT nodular at margins
Lung cancer β€” lacks the typical nodular cavity wall appearance
Bacterial abscess β€” usually more acute presentation
Why This Sign Matters
βœ“ High specificity for chronic cavitary aspergillosis
βœ“ Supports diagnosis when combined with serology and culture
βœ“ Indicates active disease β€” important for treatment decisions
βœ“ Prognostic value β€” extent may correlate with disease severity
Key Takeaway
The scab-like sign is a pathognomonic finding that significantly increases suspicion for CCPA, especially when seen in a patient with:
Chronic cavitary lung disease
Positive Aspergillus-specific IgG serology
Positive cultures from respiratory specimens
Appropriate clinical presentation (hemoptysis, cough, dyspnea)
This imaging pattern, combined with microbiological and serological confirmation, provides strong diagnostic evidence for chronic cavitary aspergillosis.
