🩺 A Tale of Two Healthcare Systems: Same Disease, Two Completely Different Fates
​As an oncologist practicing in Mangalore, India, I recently witnessed a profound coincidence that deeply shook me. Within the span of a single week, two young male patients—both in their late 20s/early 30s, both fathers to infants under a year old—consulted me for the exact same condition: Parotid Gland Cancer.
​The medical diagnosis was identical. But because of the healthcare systems they navigated, their outcomes could not be further apart.
​📉 Case 1: The UK System – A Tragic Overreliance on Protocol
​The Patient: An Indian citizen settled in the UK. His wife works as a nursing staff member within the NHS.
​The Journey: He presented with facial deviation. Following standard protocols without initial imaging, it was diagnosed as Bell’s Palsy. Trusting the system and consulting multiple doctors over 11 months, valuable time slipped away.
​The Turning Point: When he suddenly developed slurring of speech and rushed to emergency room, he was told it wasn't an acute emergency and was advised to follow up routinely.
​The Reality: An online consultation with me prompted them to fly to India immediately. Within one hour of landing, a repeat MRI in my clinic revealed the devastating truth: Stage 4 cancer with intracranial extension and bone metastasis. Cure is no longer possible.
​📈 Case 2: The Indian System – Rapid, Accessible, and Life-Saving
​The Patient: A local auto-rickshaw driver in Mangalore.
​The Journey: He noticed a tiny swelling and minor lip tingling. He came to my clinic.
​The Turnaround: I advised a simple neck ultrasound (costing less than ₹1,000). The radiologist spotted a tumor and immediately performed a needle biopsy in the same sitting. Within 4 hours, cancer was confirmed. A same-day MRI showed early-stage localized disease.
​The Outcome: The patient held a BPL card. We issued a referral letter, received Government approval under the Ayushman Bharat scheme within 48 hours, and performed surgery the next day. The entire process cost him virtually nothing. Margins and lymph nodes are completely negative—he is cured. He was back to driving his auto just three days post-surgery.
​🧠 The Medical & Personal Dilemma
​Often, Indian healthcare faces severe criticism for being commercialized or chaotic compared to Western models. Yet, this incidence highlights the unmatched agility, clinical autonomy, and speed of the Indian private-public diagnostic ecosystem. What takes a day in Mangalore took nearly a year in the UK.
​But the story doesn't end there.
​As doctors, we are human. Moved by the UK family’s tragic delay, I spent over 40 minutes in consultation, opened up my personal email to review restricted NHS documents, handled multiple off-hours calls, and coordinated urgent next-day PET-CT and biopsy slots.
​The next day, they simply didn't show up. No text, no courtesy call, leaving an incredibly expensive radioactive nucleotide slot wasted.
​💬 My Reflection
​Was I wrong to step completely out of my professional boundary to help a family in distress? No, because compassion is the bedrock of oncology. However, empathy must be a two-way street.
​While we look at structural system failures (like the one that cost a young father his chance at a cure), we must also look at communication failures. As doctors, we are happy to give you our time, our energy, and our emotional bandwidth far beyond the ₹600 consultation fee—all we ask in return is basic professional courtesy and communication.
​Early detection saves lives. But a responsive system and open communication make it possible.
