Cases That Changed Me as a Medical Oncologist

Early in my career, I believed that if I followed every guideline perfectly, every patient would receive the best possible care.

Then I met a patient who challenged that belief.

The diagnosis was straightforward.

The guidelines clearly recommended one treatment approach.

On paper, the decision seemed easy.

But the patient sitting in front of me wasn’t “on paper.”

She was elderly.

She had multiple medical conditions.

She lived alone.

And her greatest wish wasn’t simply to live longer.

It was to remain independent and spend meaningful time with her family.

Following the guideline exactly might have offered a small medical advantage.

But it also carried a high risk of side effects that could take away the quality of life she valued most.

So we paused.

We talked.

We listened.

Together, 𝘄𝗲 𝗰𝗵𝗼𝘀𝗲 𝗮 𝘁𝗿𝗲𝗮𝘁𝗺𝗲𝗻𝘁 𝗽𝗹𝗮𝗻 𝘁𝗵𝗮𝘁 𝗯𝗮𝗹𝗮𝗻𝗰𝗲𝗱 𝗲𝘃𝗶𝗱𝗲𝗻𝗰𝗲 𝘄𝗶𝘁𝗵 𝗵𝗲𝗿 𝗽𝗲𝗿𝘀𝗼𝗻𝗮𝗹 𝗴𝗼𝗮𝗹𝘀 𝗮𝗻𝗱 𝗼𝘃𝗲𝗿𝗮𝗹𝗹 𝗵𝗲𝗮𝗹𝘁𝗵.

Months later, she told me,

“Doctor, thank you for treating me—not just my cancer.”

That sentence has stayed with me ever since.

Clinical guidelines are invaluable.

They are built on years of research and help us make informed decisions.

But they are not substitutes for clinical judgment or meaningful conversations with patients.

Because every cancer is different.

𝗘𝘃𝗲𝗿𝘆 𝗽𝗮𝘁𝗶𝗲𝗻𝘁 𝗵𝗮𝘀 𝗱𝗶𝗳𝗳𝗲𝗿𝗲𝗻𝘁 𝗱𝗶𝘀𝗲𝗮𝘀𝗲 𝗯𝗶𝗼𝗹𝗼𝗴𝘆. 𝗧𝗵𝗲 𝗱𝗶𝘀𝗲𝗮𝘀𝗲 𝗶𝘁𝘀𝗲𝗹𝗳 𝘃𝗮𝗿𝗶𝗲𝘀 𝗳𝗿𝗼𝗺 𝗽𝗲𝗿𝘀𝗼𝗻 𝘁𝗼 𝗽𝗲𝗿𝘀𝗼𝗻. 𝗜𝘁 𝗶𝘀 𝗻𝗼𝘁 𝗮𝗹𝘄𝗮𝘆𝘀 𝗽𝗼𝘀𝘀𝗶𝗯𝗹𝗲 𝘁𝗼 𝘁𝗿𝗲𝗮𝘁 𝗮𝗹𝗹 𝗽𝗮𝘁𝗶𝗲𝗻𝘁𝘀 𝘄𝗶𝘁𝗵 𝗮 𝗼𝗻𝗲-𝘀𝗶𝘇𝗲-𝗳𝗶𝘁𝘀-𝗮𝗹𝗹 𝗮𝗽𝗽𝗿𝗼𝗮𝗰𝗵.

Every patient is different.

And sometimes, the best treatment is the one that fits the person—not just the disease.

That experience taught me one of the most important lessons in oncology:

Evidence guides us.

Patients lead us.

The art of medicine lies in bringing the two together.

Cases That Changed Me as a Medical Oncologist

He was responding well to treatment.

The scans were encouraging.

His symptoms were improving.

For the first time in months, there was hope.

Then one day, he didn't return for his scheduled chemotherapy.

A few weeks later, he came back.

His cancer had progressed.

I asked him gently,

"What happened?"

He looked down and said,

"𝗗𝗼𝗰𝘁𝗼𝗿... 𝗜 𝘄𝗮𝗻𝘁𝗲𝗱 𝘁𝗼 𝗰𝗼𝗻𝘁𝗶𝗻𝘂𝗲. 𝗕𝘂𝘁 𝗜 𝗰𝗼𝘂𝗹𝗱𝗻'𝘁 𝗮𝗳𝗳𝗼𝗿𝗱 𝗶𝘁 𝗮𝗻𝘆𝗺𝗼𝗿𝗲."

Those words stayed with me.

He hadn't stopped because of pain.

He hadn't stopped because of side effects.

𝗛𝗲 𝘀𝘁𝗼𝗽𝗽𝗲𝗱 𝗯𝗲𝗰𝗮𝘂𝘀𝗲 𝗵𝗲 𝗵𝗮𝗱 𝘁𝗼 𝗰𝗵𝗼𝗼𝘀𝗲 𝗯𝗲𝘁𝘄𝗲𝗲𝗻 𝘁𝗿𝗲𝗮𝘁𝗶𝗻𝗴 𝗵𝗶𝘀 𝗰𝗮𝗻𝗰𝗲𝗿 𝗮𝗻𝗱 𝗽𝗿𝗼𝘃𝗶𝗱𝗶𝗻𝗴 𝗳𝗼𝗿 𝗵𝗶𝘀 𝗳𝗮𝗺𝗶𝗹𝘆.

As oncologists, we often focus on scans, blood reports, and treatment plans.

But sometimes the biggest obstacle isn't the disease.

It's the financial burden that comes with it.

That day reminded me that caring for a cancer patient means understanding the person behind the diagnosis.

Because no one should have to choose between their life and their livelihood.

Cancer doesn't just affect the body.

It affects families, dreams, and financial security.

Sometimes, the most important conversation isn't about the next cycle of chemotherapy.

It's about making sure the patient can continue the journey.

Cases That Changed Me as a Medical Oncologist

She had recently been diagnosed with cancer.

It was an emotional time for the entire family.

As we discussed treatment, another concern emerged.

As part of the routine donor evaluation, she underwent a series of detailed medical tests.

Further investigations confirmed that she had an early liver condition that required treatment.

One sister was fighting cancer.

It reminded me that medicine is full of unexpected moments.

Together with a multidisciplinary team—including obstetricians, breast surgeons, radiologists, and neonatologists—we carefully reviewed every option.

Every investigation.

Every treatment.

Every timing decision.

Not only for the mother...

But for the baby she was carrying.

Some treatments could be given safely during certain stages of pregnancy.

Others had to wait.

Every step required balancing the mother's health with the baby's safety.

There was no perfect answer.

She was only in her thirties.

A loving mother.

Always putting her family first.

Months earlier, she had noticed a lump.

She wanted to get it checked.

Household responsibilities.

𝗜𝘁 𝗶𝘀 𝗼𝗻𝗲 𝗼𝗳 𝘁𝗵𝗲 𝗴𝗿𝗲𝗮𝘁𝗲𝘀𝘁 𝗴𝗶𝗳𝘁𝘀 𝘆𝗼𝘂 𝗰𝗮𝗻 𝗴𝗶𝘃𝗲 𝘆𝗼𝘂𝗿 𝗳𝗮𝗺𝗶𝗹𝘆.

Because they need you—healthy, present, and by their side.

If you notice a lump, unexplained bleeding, persistent weight loss, or any symptom that doesn't feel right, don't ignore it.

Early diagnosis doesn't guarantee an easy journey.

But it often gives us more treatment options and a better chance of successful outcomes.

Please don't wait for the "right time."

"𝗪𝗵𝗲𝗻 𝗧𝗼𝘅𝗶𝗰𝗶𝘁𝘆 𝗕𝗲𝗰𝗮𝗺𝗲 𝗠𝗼𝗿𝗲 𝗗𝗮𝗻𝗴𝗲𝗿𝗼𝘂𝘀 𝗧𝗵𝗮𝗻 𝗧𝗵𝗲 𝗖𝗮𝗻𝗰𝗲𝗿"

Cases that changed me as a Medical Oncologist.

There are moments in oncology when the biggest threat is no longer the cancer.

This was one of them.

The patient had advanced cancer.

We had started treatment with a clear goal—to control the disease and improve quality of life.

Initially, things seemed encouraging.

The scans were stable.

The cancer was responding.

Everyone felt relieved.

Then things began to change.

The patient became weaker.

Simple daily activities became exhausting.

Hospital visits became more frequent.

Blood reports became increasingly concerning.

At first, it was easy to blame the cancer.

But as we looked closer, another reality emerged.

The treatment was causing more harm than benefit.

The very therapy meant to help was now taking away the quality of life we were trying to preserve.

The family asked the question many families ask:

"Doctor, shouldn't we continue? What if stopping allows the cancer to grow?"

It was a difficult conversation.

Because medicine often teaches us how to start treatment.

But experience teaches us when to stop.

So we paused.

We reassessed.

And together, we made a decision that felt uncomfortable at first.

𝗪𝗲 𝘀𝘁𝗼𝗽𝗽𝗲𝗱 𝘁𝗿𝗲𝗮𝘁𝗺𝗲𝗻𝘁.

Not because we were giving up.

But because the patient's wellbeing mattered more than the treatment itself.

Over the following weeks, something remarkable happened.

The patient felt better.

Energy returned.

Appetite improved.

Time was spent with family instead of in hospital beds.

That experience reinforced a lesson I carry into every consultation:

𝗧𝗵𝗲 𝗴𝗼𝗮𝗹 𝗶𝘀 𝗻𝗼𝘁 𝘀𝗶𝗺𝗽𝗹𝘆 𝘁𝗼 𝘁𝗿𝗲𝗮𝘁 𝗰𝗮𝗻𝗰𝗲𝗿.

𝗧𝗵𝗲 𝗴𝗼𝗮𝗹 𝗶𝘀 𝘁𝗼 𝗵𝗲𝗹𝗽 𝘁𝗵𝗲 𝗽𝗮𝘁𝗶𝗲𝗻𝘁.

And sometimes those are not the same thing.

One of the most important principles in oncology is this:

"𝗧𝗵𝗲𝗿𝗮𝗽𝘆 𝗰𝗮𝗻 𝗻𝗲𝘃𝗲𝗿 𝗯𝗲 𝗺𝗼𝗿𝗲 𝘁𝗼𝘅𝗶𝗰 𝘁𝗵𝗮𝗻 𝘁𝗵𝗲 𝗱𝗶𝘀𝗲𝗮𝘀𝗲 𝗶𝘁𝘀𝗲𝗹𝗳."

When treatment stops helping the person in front of us, we must have the courage to rethink our approach.

Because good cancer care is not about doing more.

It's about doing what is right.

Every treatment decision should balance effectiveness, safety, and quality of life. Sometimes the bravest medical decision is knowing when to change course.

𝗔 𝗗𝗿𝗮𝗺𝗮𝘁𝗶𝗰 𝗥𝗲𝘀𝗽𝗼𝗻𝘀𝗲 𝘁𝗼 𝗜𝗺𝗺𝘂𝗻𝗼𝘁𝗵𝗲𝗿𝗮𝗽𝘆 𝗜 𝗗𝗶𝗱 𝗡𝗼𝘁 𝗘𝘅𝗽𝗲𝗰𝘁.

Cases that changed me as a Medical Oncologist.

Some cases remind you why medicine continues to surprise us.

A few years ago, I met a patient whose cancer had already spread.

The diagnosis was aggressive.

The scans were concerning.

The family had spent weeks searching for answers and hoping for options.

When we discussed treatment, immunotherapy was one of them.

There was a scientific reason to believe it could help.

But if I'm honest, my expectations were measured.

In oncology, we learn to be hopeful—but realistic.

Treatment began.

A few weeks later, the patient returned for follow-up.

Clinically, he looked better.

His energy had improved.

Symptoms that had troubled him for months were beginning to settle.

We were encouraged.

But we waited for the scans.

When the reports arrived, I opened them expecting improvement.

What I saw surprised me.

The tumors had shrunk significantly.

Far more than I had anticipated.

I reviewed the images again.

Then compared them with the previous scans.

The difference was remarkable.

For the patient and family, it felt like a weight had been lifted.

For me, it was a reminder of why cancer treatment continues to evolve.

Not every patient responds this way.

Not every cancer is suitable for immunotherapy.

And not every story has the same ending.

But sometimes, when the biology is right and the treatment is matched carefully to the patient, the results can exceed expectations.

That patient taught me an important lesson:

𝗡𝗲𝘃𝗲𝗿 𝘂𝗻𝗱𝗲𝗿𝗲𝘀𝘁𝗶𝗺𝗮𝘁𝗲 𝘁𝗵𝗲 𝗽𝗼𝘄𝗲𝗿 𝗼𝗳 𝗺𝗲𝗱𝗶𝗰𝗮𝗹 𝗽𝗿𝗼𝗴𝗿𝗲𝘀𝘀.

Behind every new treatment is the possibility of giving someone more time, more moments, and more hope.

As oncologists, we celebrate every response.

But the unexpected ones stay with us forever.

Every patient is unique. Treatment decisions should always be individualized based on the type of cancer, stage, and clinical evaluation.

Cases That Changed Me as a Medical Oncologist.

Everything seemed straightforward.

The scans were concerning.

The symptoms fit.

The diagnosis appeared clear.

The patient and family had prepared themselves for the journey ahead.

We had discussed chemotherapy.

Reviewed the treatment plan.

Answered questions.

The next step seemed obvious.

Then one report arrived.

A report we had been waiting for.

And suddenly, everything changed.

The new findings challenged our assumptions.

The disease was not behaving the way we initially thought.

What looked like one condition was actually something entirely different.

Had we rushed ahead, the patient might have received a treatment they didn't need.

Instead, we paused.

Reviewed everything again.

Discussed the findings with colleagues.

Looked beyond the obvious.

And changed the plan completely.

The patient never received the chemotherapy we had initially prepared for.

Because it was no longer the right treatment.

That day reminded me of one of the most important lessons in oncology:

𝗚𝗼𝗼𝗱 𝗱𝗼𝗰𝘁𝗼𝗿𝘀 𝗱𝗼𝗻'𝘁 𝗷𝘂𝘀𝘁 𝗸𝗻𝗼𝘄 𝘄𝗵𝗲𝗻 𝘁𝗼 𝘁𝗿𝗲𝗮𝘁.

𝗧𝗵𝗲𝘆 𝗸𝗻𝗼𝘄 𝘄𝗵𝗲𝗻 𝘁𝗼 𝘀𝘁𝗼𝗽, 𝗿𝗲𝗮𝘀𝘀𝗲𝘀𝘀, 𝗮𝗻𝗱 𝗮𝘀𝗸 𝗺𝗼𝗿𝗲 𝗾𝘂𝗲𝘀𝘁𝗶𝗼𝗻𝘀.

Medicine is not about being fast.

It's about being right.

And sometimes, the most important treatment decision is the decision to wait for one more answer.

The patient did well.

Not because we acted quickly.

But because we acted carefully.

That experience reinforced something I tell my patients often:

A diagnosis should guide treatment. Assumptions should not.
