# # # **Retinoscopy Matters More Than You Think**

Many parents think, **“If my child can’t sit on the big machine, we can’t check their number.”**

That’s not true.

A child doesn’t always need to sit perfectly on an autorefractor or other machine for us to understand their refractive status. **Retinoscopy is one of the most important clinical skills in optometry**, especially when examining children, young patients, or patients who cannot reliably respond to subjective testing.

Retinoscopy allows the clinician to objectively assess the eye’s refractive status by observing the eye’s response to light—and it can provide valuable information even when the patient cannot communicate clearly or cooperate with conventional testing.

But remember: **retinoscopy is not simply about finding a spectacle power.** A skilled clinician interprets the findings in the context of the patient’s visual behaviour, accommodation, binocular vision and overall visual function.

So if your child cannot sit on a machine, **don’t assume that their vision cannot be properly assessed.**

Look for an **optometrist or eye-care professional who is experienced in retinoscopy and pediatric/functional vision assessment.**

Because sometimes, the most important instrument in the room isn’t the biggest machine—it’s the **clinician’s skill.** 👁️

**If your number is increasing, then this is for you. 👁️📈**

When your spectacle number keeps increasing, simply changing the prescription every few months is **not always the complete solution.**

A changing prescription is a **sign** that deserves further evaluation—not just another pair of glasses.

To understand what is really happening, we need to look beyond visual acuity and refraction.

A detailed myopia evaluation may include:

• Refraction and visual acuity
• Binocular vision and accommodative function
• Ocular health examination
• Corneal parameters
• **Axial length measurement**
• Lifestyle and near-work habits
• Screen exposure and outdoor time
• Family history and age of onset
• Previous rate of myopia progression

**Axial length is particularly important** because increasing eye length is one of the major structural changes associated with myopia progression. Tracking it over time can give us valuable information about whether the eye is continuing to elongate.

Once we understand **why the prescription is increasing and how the eye is changing**, we can choose a more appropriate management strategy rather than simply prescribing stronger glasses.

Myopia management is not just about making you see 20/20.

# # # **It is about understanding the eye, monitoring progression, and protecting vision for the future.**

If you or your child has a rapidly increasing prescription, don’t wait until the number becomes very high.

**Get a comprehensive myopia evaluation and monitor the progression regularly.**

**Yes, you heard it right 👀 — squint surgery does NOT always mean the visual system is fully “fixed.”**

Many parents believe that once the eyes are surgically aligned, everything will automatically return to normal.

But here’s the important part: **eye alignment and functional vision are not the same thing.**

Squint (strabismus) surgery primarily aims to improve the **physical alignment of the eyes**. But your brain still has to learn how to use the information coming from both eyes together.

And this is especially important for **depth perception (stereopsis).** 👁️🧠

Depth perception is not simply something the eyes “have.” It is a **learned visual skill** that develops through the brain’s ability to combine information from both eyes and create a single, meaningful 3D visual experience.

After squint surgery, some patients may still have difficulty with:
• Binocular vision
• Eye teaming
• Convergence and divergence
• Eye movements and tracking
• Visual fixation
• Depth perception
• Spatial awareness
• Visual attention

This is where **Vision Therapy may play an important role.**

Think of it this way:

**Surgery can help align the eyes.
Vision Therapy can help train the visual system to use those eyes together more effectively.**

The goal isn’t simply to make the eyes *look straight*.

The goal is to help the brain develop better **binocular coordination, visual processing and functional vision.**

However, Vision Therapy is **not automatically required after every squint surgery**. Each patient needs a proper binocular and functional vision assessment to determine whether therapy is appropriate.

So if you or your child has undergone squint surgery, don’t stop at asking:

**“Are the eyes straight?”**

Also ask:

**“Can the brain use both eyes together efficiently?”** 👁️👁️🧠

Because **straight eyes are important — but functional binocular vision is the bigger picture.**

**This is not just a retinoscope. 👁️🧠**

For me, it’s a window into something much bigger — **how your brain is interpreting the 3-dimensional world around you.**

Vision is not simply about seeing clearly or reading the smallest line on an eye chart.

Your visual system is constantly processing:
👁️ Where things are
🧠 How far away they are
↔️ How they move through space
🎯 How your eyes coordinate with your body
⚡ How your brain responds to visual information

Every movement, every reflex and every visual response gives us information beyond **20/20 eyesight.**

Because **eyesight tells you how clearly you see.
Vision tells you how your brain uses what you see.**

And sometimes, the problem isn’t the eyes alone — it’s the way the **visual system and brain work together.**

That’s where a deeper visual assessment can make a difference.

**We don’t just test eyes.
We understand vision.** 🧠👁️

**Just a pair of the best glasses isn’t always enough. 👓🧠**

If you’re still experiencing headaches, eye strain, blurred vision, double vision, dizziness, poor reading, difficulty concentrating, motion sickness, or your child struggles with learning despite having the “correct” glasses, the problem may not be your eyesight—it could be your **visual system**.

Vision is much more than seeing clearly. It is how the eyes and brain work together to process, interpret, and respond to the world around us. Your visual system involves eye movements, focusing, eye teaming, visual processing, balance, posture, attention, and coordination.

Even with perfect vision (20/20), these visual skills can be underdeveloped or disrupted due to developmental delays, concussion, stroke, neurological conditions, autism, ADHD, or prolonged visual stress.

This is where a **developmental optometrist** can help. Through a comprehensive functional vision assessment, they evaluate how your entire visual system is working—not just how clearly you can read an eye chart. If needed, a personalized **Vision Therapy** program can help improve these visual skills by training the brain and eyes to work together more efficiently.

Remember:
**Clear eyesight doesn’t always mean efficient vision.**

If glasses haven’t solved your symptoms, it may be time to evaluate your **visual system**, not just your eyesight.

**Rahul’s Journey: Looking Beyond the Obvious**

Three months ago, Rahul walked into our clinic with persistent vertigo that was affecting his daily life. He had already consulted multiple healthcare professionals and had been advised medication, including antidepressants, to help manage his symptoms. Yet, he still didn’t feel like himself.

At **Alphaa Vision Therapy**, we believe every patient deserves a comprehensive evaluation. During our assessment, we identified visual dysfunctions that could have been contributing to his dizziness and balance difficulties.

We designed an individualized **Vision Therapy and Neuro-Vision Rehabilitation** program tailored to his needs. Over the following weeks, Rahul made remarkable progress. His symptoms reduced significantly, and he regained confidence in his daily activities.

This case is a reminder that **not every case of dizziness or vertigo has the same cause**. For some individuals, the visual system may play an important role and deserves to be evaluated as part of a multidisciplinary approach.

**Sometimes, the missing piece isn’t another medication—it’s understanding how the eyes, brain, and body work together.**
