ICG Camera In Breast Cancer Surgery

अब Breast Cancer Surgery में ICG Camera की मदद से Sentinel Node Biopsy करना ज्यादा आसान और safe हो गया है। Sentinel Node Biopsy करने से surgery के बाद हाथ में सूजन यानी Arm Swelling का risk काफी कम हो जाता है।

Breast Conservative Surgery And Oncoplasty

Breast Conservative Surgery में सिर्फ lump निकालना ही जरूरी नहीं होता, बल्कि उसके आसपास का Safe Margin भी निकालना पड़ता है। अगर 2 cm का lump हो और उसके चारों तरफ 1 cm का Safe Margin लिया जाए, तो Breast में एक बड़ा Defect बन सकता है। अगर इस Defect को उसी समय fill नहीं किया जाए, तो Breast Shape abnormal दिख सकती है और patient को कपड़े पहनने में भी discomfort हो सकता है।

इसीलिए उसी समय Oncoplasty की जाती है ताकि Breast Shape natural रहे और patient अपनी Body Image से happy और confident महसूस करे। Oncoplasty में Oncology और Plastic Surgery दोनों के principles follow किए जाते हैं ताकि Cancer Surgery के साथ साथ Cosmetic Result भी बेहतर मिले।

Skin Sparing Mastectomy Surgery

इस वीडियो में बताया गया है कि Skin Sparing Mastectomy को कैसे plan किया जाता है और breast cancer surgery के दौरान incision marking कितनी महत्वपूर्ण होती है। Surgeon सबसे पहले breast tissue की boundary mark करते हैं और parasternal area के perforators को ध्यान में रखते हुए surgery plan की जाती है ताकि reconstruction के लिए maximum skin preserve की जा सके।

इस patient को पहले chemotherapy दिया गया था जिसकी वजह से lump काफी छोटा हो चुका है। सामान्य mastectomy में बड़ा incision लिया जाता है ताकि पूरा breast tissue और axilla आसानी से access किया जा सके। लेकिन Skin Sparing Mastectomy में केवल affected skin remove की जाती है और बाकी breast skin बचा ली जाती है।

इससे surgery के बाद Breast Reconstruction बेहतर तरीके से किया जा सकता है। Reconstruction implant की मदद से भी किया जा सकता है और abdomen tissue का उपयोग करके भी breast shape restore किया जा सकता है। यह technique cosmetic outcome और recovery दोनों के लिए काफी beneficial मानी जाती है।

Breast Cancer Mammogram

Mammogram में सिर्फ tumor देखना ही जरूरी नहीं होता। Skin thickening, ni**le pulling और enlarged lymph nodes जैसे signs भी locally advanced breast cancer की तरफ इशारा कर सकते हैं।

ऐसे cases में पहले chemotherapy देने से patient को काफी फायदा मिल सकता है। इसी patient को भी पहले chemotherapy दी गई थी और अब mastectomy planned है। Breast cancer diagnosis में mammogram findings को सही तरीके से समझना बहुत जरूरी होता है।

53 साल की इस patient को Breast Lump की शिकायत थी। Systematic investigations में X Ray Mammography, Core Biopsy और PET Scan किया गया। Reports में पता चला कि patient को Single Breast Cancer Lump है जो ER PR Strong Positive है और body में कहीं भी metastasis नहीं है।

ऐसे cases में Breast Conservation Surgery एक safe और effective option होता है। साथ में Oncoplasty करने से breast shape, cosmetic result और self body image भी काफी बेहतर रहता है।
