A question I’m often asked is, “Is India facing a fertility crisis?”

My answer is that I don’t think this is limited to India. Fertility is declining across the globe.

One thing I’ve noticed over the years is that the WHO guidelines for normal semen parameters have changed multiple times. And with every update, the benchmark for what is considered “normal” has only declined.

There isn’t a single reason for this, but one possible contributor is our increasing exposure to endocrine disruptors in everyday life.

Some common sources include:
• Pesticides in our food
• Makeup and cosmetic products
• Mobile phones
• Laundry detergents
• Perfumes
• Scented candles

Understanding the environment around us is an important part of understanding fertility.

One of the most common questions during an IVF journey is understanding embryo grades. While terms like ‘5AA, 5BB, or 4AA’ may seem confusing, they’re simply a way for embryologists to describe how an embryo looks under the microscope before transfer.

The ‘Gardner Grading System’ evaluates three important aspects:

Expansion – How developed the blastocyst is.

Inner Cell Mass (ICM) – The group of cells that will develop into the baby.

Trophectoderm (TE) – The outer layer of cells that will form the placenta.

Higher grades often indicate better morphology, but an embryo’s appearance is only one piece of the puzzle. Implantation and a healthy pregnancy also depend on several factors, including the embryo’s genetic health, the uterine environment, maternal age, and overall reproductive health.

Remember, an embryo grade is a guide—not a guarantee. Beautiful babies have been born from embryos with a wide range of grades, and every embryo deserves hope.

Understanding your embryo grading helps you make informed decisions with your fertility specialist and approach your IVF journey with confidence.

For many women, this is the first time they hear the term during a routine transvaginal scan at a fertility consultation. Naturally, it can feel worrying—but it doesn’t automatically mean your fertility journey has become more difficult.

An endometriotic cyst, also known as an ovarian endometrioma, is a cyst caused by endometriosis. Depending on its size, location, your symptoms, and its impact on ovarian reserve or fertility, your fertility specialist will recommend the most appropriate next steps.

The important thing to remember is that not every endometriotic cyst requires surgery, and not every woman with an endometrioma will have difficulty achieving pregnancy. Treatment is always individualised, with the goal of preserving both your health and your fertility whenever possible.

A fertility consultation isn’t just about identifying a diagnosis—it’s about understanding what that diagnosis means for you and creating a personalised plan based on your goals.

Because the right information doesn’t create fear—it gives you the confidence to make informed decisions.

The days after an embryo transfer are often the hardest part of the IVF journey. They’re filled with hope, anticipation, and a lot of questions about what’s happening inside your body.

Once the embryo is transferred into the uterus, its journey is only just beginning. Depending on whether a Day 3 or Day 5 embryo was transferred, the timeline may vary slightly. Over the next few days, the embryo hatches from its outer shell, attaches to the uterine lining, and, if implantation is successful, begins establishing the connection needed to support an ongoing pregnancy. As implantation progresses, the body starts producing beta hCG, the hormone detected in a pregnancy test.

It’s also important to remember that mild cramping, fatigue, breast tenderness—or even having no symptoms at all—can all be completely normal. Symptoms alone cannot tell us whether implantation has occurred.

Every embryo follows its own timeline, and every fertility journey is unique. During this phase, patience, self-care, and following your fertility specialist’s advice are the best things you can do.

Sometimes, the biggest milestones happen quietly—one day at a time.

Needle-free IVF is something you’ve probably come across if you’ve been following fertility content on Instagram. But what does it actually mean?

There are two types of needle-free IVF:
• A wearable device that is attached to the front of the abdomen. It has a tiny needle placed in the subcutaneous tissue, through which gonadotropin injections are delivered.
• A high-pressure device that delivers the same gonadotropins into the subcutaneous tissue without using a conventional needle.

While both are good options for people who have a fear of needles, I believe IVF injections don’t have to be scary when taken the right way.

Here’s what I recommend:
• Apply an ice pack to the injection site for about 1 to 1½ minutes to numb the area.
• Pinch the skin firmly before giving the injection.
• Inject slowly.
• Apply ice to the area again after the injection.

Whether you choose a needle-free option or a conventional injection, the right technique can make all the difference.

Sending ‘them’ straight to the sidelines.

Here’s the thing: IVF isn’t about replacing nature, it’s about supporting it.

IVF is a form of assisted conception that helps fertilization occur when spontaneous conception isn’t possible or is taking longer than expected. Once an embryo is transferred into the uterus, the pregnancy continues just like any other pregnancy.

The body still does the incredible work of implantation, growth, and nurturing new life.

Calling IVF “unnatural” overlooks the science, the dedication, and the hope behind every treatment cycle. More importantly, it dismisses the journeys of millions of individuals and couples who rely on assisted conception to build their families.

So yes…

Saying “IVF is unnatural”?

It may seem surprising, but checking embryos less often is actually part of giving them the best chance to grow.

Embryos are typically assessed at key milestones—Day 1, Day 3, and Day 5—because these checkpoints provide the most meaningful information about their development. Opening the incubator or handling embryos more frequently can disturb the stable environment they need to thrive.

Another important point many patients don’t know is that not every healthy embryo becomes a blastocyst on Day 5. Some naturally reach this stage on Day 6 or even Day 7, and many of these embryos can still lead to healthy pregnancies.

Embryo development isn’t a race. Every embryo follows its own biological timeline.

Our goal isn’t to check embryos more often—it’s to create the best possible conditions for them to reach their full potential.
