The ending month of this year was the first month of my new begining, My own orthopedic setup. Its been two years since I left KEM hospital and Mumbai city, searching for a place where i am needed the most. Lots of ups and downs, struggle, meeting hundreds of new people, working with minimum resources,... it was really a major learning phase of my life. I got Lots of love and support from my family, my wellwishers, my daughter. The journey is endless. Yet The beginning of the end of the end of the beginning has begun...

A well done MIPO plating of distal tibia is even less invasive than nailing as we don't touch the extensor mechanism. Also we can achieve best anatomical reduction using skeletal traction intraoperatively using a pin in distal fragment to manipulate rotation at fracture site. In this 56 year old female, even for proximal screws I used six tiny incisions just enough to pass a locking sleeve and a screw head for preserving most of the natural soft tissue cover. Knee and ankle was mobilised the next day and toe touch weight bearing started as I was confident with the fixation. Clinical and x ray photos at 6 weeks are also shared.

3 weeks old lateral condyle fracture with PCL avulsion. Treated with open reduction and screws on lateral condyle by swashbuckler approach keeping lateral attachments on epicondyle intact. For avulsion anchor with suture bridge pulled through a tunnel and fixed anteriorly with a disc. Suture bridge using anchor was done first time by me and it really provided excellent buttress of the small bone fragment which being so small, was not fixable with a screw. For PCL posterior Burks approach was used with L shaped incision.

A rare case of recurrent shoulder dislocation associated with old coracoid fracture which was discovered in pre-op CT Scan (done to calculate the glenoidal bone loss). Same coracoid was used as a bone block to prevent recurrent shoulder dislocation. Since it was small compared to the normal graft taken in a patient without any fracture, I was skeptical about its healing. But contrary to the expectation, it healed well as seen in CT Done 2 months post op. Sharing the clinical and radiological images of the same. Clinical images are At 2 months after the surgery. Expecting to get more external rotation in coming months.

32 year female with radial head and capitellum fracture dislocation ORIF. Sharing results at 2 months post op.

A case of recurrent shoulder dislocation. 19 year old Kabaddi player with history of 10 to 12 episodes of shoulder dislocation with more than 20 percent glenoid bone loss (as calculated on software using maximum diameter circle method). he had quit playing Kabaddi for his problem.

I operated him for congruent arc latarjet surgery last year. Patient got almost full range at 7 months follow up and is now back to playing kabaddi. CT scan was done at around 3 months, and the images show complete union of coracoid graft (shared below).

26 yr old male patient with talus fracture dislocation and medial plus posterior malleolus fracture. Reduced with intraoperative use of skeletal pin traction and fixed with two CC screws under C arm. Both medial and posterior malleolus were also fixed with two and one CC screw respectively
