So many patients keep on trying naturally and are not able to conceive with such a picture in the abdomen.
The patient was referred with anterior abdominal wall adhesions.
On putting a laparoscope, we found such flimsy adhesions throughout the abdomen: TYPICAL of abdominal Koch's.
As a surgeon, it is very important to know when to NOT operate. So we did a little adhesiolysis, took a peritoneal biopsy (she had no previous H/o TB) and informed the attendants.
Ge***al and abdominal Koch's is notoriously difficult to treat. In a country like India, unfortunately, an infectious disease like TB is still the cause of much morbidity and mortality. The awareness is low and the diagnosis is even lower. In such cases, best to be completely transparent with your patients wrt the findings.
