3 IVF failures. Good-quality eggs. But poor-quality embryos.

A couple came to me after three unsuccessful IVF attempts at reputed IVF centres in Mumbai.

The wife had been told that her egg quality was good, but the embryo quality was poor.

So the important question was:

What could be contributing to the poor embryo development?

When we reviewed the couple's history in detail, we identified an important area that had not previously been evaluated: the husband's s***m parameters and DNA fragmentation.

A DFI, or DNA Fragmentation Index, test showed a high level of s***m DNA fragmentation in his case.

The couple was then managed with a three-month treatment plan, followed by IVF with additional laboratory techniques, including microfluidics for s***m selection and ICSI.

In this particular case, all 5 eggs fertilized and 4 top-quality blastocysts were obtained.

After embryo transfer, the patient conceived with twins and was doing well at 18 weeks of pregnancy when this case was shared.

This case highlights an important point:

When IVF fails repeatedly, it is important to look beyond the woman's egg quality alone.

Egg quality, s***m parameters, s***m DNA integrity and embryo development can all be relevant when evaluating a couple.

If you have experienced repeated IVF failures, consider taking a second opinion and reviewing your previous IVF history in detail with a fertility specialist.

Every case is different, and the appropriate evaluation and treatment should be individualized.

IVF is always the last resort. Is that really true?

Not always.

There are certain fertility conditions where IVF may be recommended earlier, rather than being kept as the final option.

For example:

* Severely blocked fallopian tubes
* Severe endometriosis with distorted pelvic anatomy
* Very low or zero s***m count in certain cases
* Very poor ovarian reserve
* When previous fertility treatments have not been successful

In some of these situations, continuing to wait or repeatedly trying treatments that are unlikely to work may only delay appropriate care.

The right treatment depends on the cause of infertility, the age of the couple, previous treatment history and the overall clinical picture.

So don't assume that IVF should always come at the end of the fertility journey.

What is the difference between IVF and ICSI?

Both are assisted fertility treatments, but the way fertilization happens is different.

In conventional IVF, eggs and s***m are placed together in a laboratory, allowing the s***m to fertilize the egg naturally.

With ICSI, one suitable s***m is carefully selected and injected directly into the egg by an embryologist.

ICSI may be considered in certain situations, such as:

* Very low s***m count
* Abnormal s***m morphology
* Use of frozen s***m
* S***m retrieved through TESA or micro-TESE
* Previous fertilization failure
* Certain cases of severely poor s***m parameters

However, ICSI does not guarantee fertilization or pregnancy.

The right approach depends on your s***m parameters, fertility evaluation, previous treatment history and individual clinical situation.

So, should you have IVF or ICSI?

That decision should be made with your fertility specialist based on your specific case.

Recurrent Implantation Failure: Why Does It Happen?

When good-quality embryos are transferred into a seemingly suitable uterine lining, but implantation does not occur repeatedly, it can be one of the most challenging situations in fertility treatment.

The important question is not just “Why did IVF fail?”

It is: “What could be the underlying reason?”

A fertility specialist may need to evaluate several factors, including:

* Embryo quality and genetic factors
* Uterine factors such as polyps, fibroids or a septum
* Endometrial inflammation or infection
* Hormonal and metabolic conditions
* S***m quality and other male fertility factors
* Previous IVF history and embryo grading
* Findings from previous investigations or procedures

In selected cases, additional testing or advanced laboratory techniques may also be considered.

The goal is to understand whether the issue is related to the embryo, the uterus, or another underlying factor, and then plan treatment accordingly.

Recurrent implantation failure does not mean that you should simply give up.

Male Fertility: Why Hormone Tests Matter

When a couple is struggling to conceive, semen analysis is an important part of male fertility evaluation. But sometimes, we need to look beyond the semen report.

Baseline hormone testing can provide valuable information about hormonal factors that may affect s***m production and reproductive health.

Depending on the individual case, your fertility specialist may recommend tests such as:

* FSH
* Testosterone
* Thyroid
* Prolactin

If any of these are abnormal, it may help your doctor understand what could be affecting the semen parameters and decide what further evaluation may be appropriate.

Fertility evaluation should not focus only on the woman.

The husband’s health and hormonal profile matter too.

What is the success rate of IVF?

This is one of the first questions couples ask when they begin their IVF journey.

But there is no single success rate that applies to every couple.

IVF outcomes can depend on several factors, including:

* Age of the couple
* Egg quality
* S***m quality
* Embryo quality
* Endometrial lining
* Genetic factors
* Embryology laboratory conditions
* Expertise of the doctor and embryologist
* Individualized treatment planning

IVF also does not guarantee a pregnancy in the first attempt. Some couples may require more than one treatment cycle.

At the same time, it is important not to assume that IVF success is simply “20–30% and the rest is luck.” Advances in reproductive medicine, embryology, genetic testing and individualized treatment have changed what is possible for many patients.

The most important thing is to understand that your IVF journey is individual.

Don't expect a 100% guarantee. But don't lose hope because of a generic success-rate number either.

Is Embryo Grade the Only Factor That Matters in IVF?

Receiving an embryo report can naturally bring many questions, especially when you see terms such as Grade A, B, or C.

Embryo grading is one way embryologists describe certain features of an embryo, such as its appearance and stage of development. However, embryo grade is only one factor considered when assessing an embryo in the context of IVF.

Depending on the individual situation, fertility teams may also consider factors such as:

• Embryo characteristics and development
• Age and reproductive history
• Genetic testing, when appropriate
• Uterine and endometrial factors
• Embryology laboratory practices and conditions
• The overall clinical circumstances

Importantly, embryo grade alone cannot predict with certainty whether implantation or pregnancy will occur. A higher grade does not guarantee a successful outcome, while an embryo with a lower grade does not necessarily mean that pregnancy is impossible.

If your report mentions Grade B or Grade C, try not to draw conclusions from the grade alone. Your fertility specialist can help explain what the findings may mean in the context of your individual treatment.

The goal is to understand the complete clinical picture—not a single number or letter.
