Gangrenous perforated appendix in six year old girl.It is life threatening situation in anyone & more in small child. Abdomen is full of foul smelling faeculent pus.बच्चों मे सभी पेट का दर्द simple abdominal colic या nonspecific pain नहीं होता है।

Reporting time after any medical illness or complication is very important for both survival & outcome of disease.
Two picture are shown in photo frame of same patient one with red viable intestine came out on coughing of previous scar & defect of incisions hernia patient.If she had timely reported then gut could had saved & such operation is much easier to perform.Both morbidity & mortality is negligible in such patient.
In second photograph of same patient after 12 hr after incident.Gut shown is black, non viable, gangrenous requiring resection & anastomosis of gangrenous non viable gut segment which is difficult ,time taking & life threatening procedure in compromised patient with many likely complication resulting in increased morbidity & mortality.
Patient is wasting unnecessary golden time in consulting local quacks,Dai,compounder,Dalal,middlemen so called medical facilitator, dresser,doctors working like quacks & taking consultation or consent of so many people before allowing intervention.Time is money.You must understand it.

Post abdominal hysterectomy patient(2003) had incisions hernia five years before.Reair of incisional hernia was done elsewhere one & half years before. She had one small persistent defect at upper part of incision in whole post operative period with persistent discharge.Herniation occurs from that defect & because of narrow neck loop became gangrenous which required resection & end to end anastomosis of gangrenous segment. We could not put mesh in such situation of gangrenous loop .

APH (Abruptio placentae) placenta attached on lower segment of anterior wall separated prematurely leading to collection of Blood between uterine wall & central placenta with no spillage of Blood .Posterior wall is pink absolutely normal while anterior wall is completely black I.e. courvoisier's uterus.It is dangerous condition for both mother & baby because due to placental separation baby don't get sufficient Blood supply & severely lacks adequate oxygenation.Mother is also at risk because it is concealed haemorrhage.It means that there is intrauterine haemorrhage but it is not coming out or patient does not present with vaginal bleeding early. Presentation is usually late & life threatening in this condition.
