Anterior parotid approach or mini parotid approach is a novel technique to treat condylar fractures of Mandible .
This is a case of delayed presentation of fracture with a history of RTA 25 days back with severe trismus ( difficulty to open mouth ) and malocclusion.
After a week of conservative management with exercises and muscle relaxants with no improvement Patient was counselled to undergo surgery by open reduction and fixation of the fracture .
Personally feel this approach is versatile to treat most fractures of condyles .
Here I present the sequence to show this minimally invasive technique which utilises a small 2.5 cm incision with minimum scar and great access to give very satisfactory mouth opening and occlusion.

Ameloblastoma s are rare tumours that grow in the jaws .
Though they are benign in nature they tend to destroy a significant part of the jaws .

These tumours need early intervention which can include local curettage , en block resections , marginal resections or hemi-mandibulectomies followed by reconstruction of jaws by bone grafts .

Here I present a case of of right posterior mandible that was operated by block resection and reconstructed using bone harvested from the hip ( anterior iliac crest ) .

Surgical success is always about teamwork .
For me it’s all about our surgical assistants , our scrub nurses , our Anestheists and last but not the least our theatre support workers .

Following up on the post of the geriatric mandible fracture .

Birds eye view of what we see when we operate .

It’s not always open skin put plate and screw and come out .

Note the amount of time it takes to fix just a small plate and two screws which sometimes take repeated reassessing and readjusting .

Geriatric Maxillofacial injuries .

This is the Oldest patient I have personally treated under general Anesthesia and pleased to have made a full recovery !!!

Some of the hardest surgeries to preform are that involving the extremes of ages !!!

I am presenting a case of a 96 year old gentlemen with senile dementia who had fractured his mandible by alleged fall from his bed .

The patient unfortunately presented with completely edentulous jaw (teeth less) with no prior use of dentures Which made gunning splints or other closed reduction not a viable option in this case .

Note - Open reduction was planned with minimum stripping of periosteum with small incisions to maintain the blood supply attempting the so called “supraperiosteal / mucosal fixation !!!

Frontal bone approached and reconstructed using titanium mesh and mini plates through pre-existing wound on his forehead .
This one was extremely challenging to reduce and fix . The access was limited by the wound and I wanted to avoid extending the scar and also the fact that the bones were so severely fractured , displaced and depressed made it even more difficult!!
Overall results was better than i expected .
