Hope Nkomamba Nankamba from Lusaka, Zambia, had been suffering from severe Right Hemifacial Spasm for nearly 10–12 years. The condition caused involuntary spasms on the right side of her face, making it difficult for her to comfortably keep her eye open. Over time, these persistent facial spasms significantly affected her confidence, daily activities, and quality of life.

Despite consulting multiple hospitals in Zambia, she could not find lasting relief. On recommendation, she visited IBS Hospital, India, where she was evaluated by Dr. Vikas Gupta and Dr. Sachin Kandhari.

After detailed evaluation, she was diagnosed with Right Hemifacial Spasm due to a vascular loop compressing the VII/VIII nerve complex — a condition in which an aberrant blood vessel compresses the facial nerve at the root exit zone, leading to persistent facial twitching and spasms.

Advanced Surgical Management Performed:

• Right retrosigmoid craniotomy
• Microsurgical Microvascular Decompression (MVD)
• Careful separation of the aberrant vascular loop from the VII/VIII nerve complex
• Teflon pledget placement between the vascular loop and nerve complex

Intraoperatively, it was found that the aberrant artery was densely adherent to and looping around the nerve complex, causing compression at multiple points, making it a rare and technically challenging case.

Remarkably, following surgery, her facial spasms reduced significantly by nearly 70–80%. She was finally able to keep her eye open comfortably again and regained the confidence to return to her normal daily activities.

Hemifacial Spasm should not be ignored, especially when symptoms persist despite prolonged medical management. Early diagnosis and advanced surgical treatment such as Microvascular Decompression (MVD) can provide long-term symptomatic relief and significantly improve quality of life.

• Separation of the superior cerebellar artery vascular loop compressing the nerve
• Duroplasty using Duragen

During surgery, it was found that the superior cerebellar artery loop was bisecting the trigeminal nerve, making it a rare and complex case.

Remarkably, after surgery, she experienced dramatic relief from the severe pain that had troubled her for years. She was able to speak and chew comfortably again without pain.
