Interesting case of laparoscopic cholecystectomy in the patient with large liver cyst in segment 4. Patient had undergone ERCP 20 days ago for multiple CBD stones. It required immense patience while dissecting calot's triangle and seperating GB from liver bed, actually there was no liver posterior to the gallbladder, it was replaced by the cyst. Operation was completed without rupturing the cyst. Complications that can arise are haemorrhage and Bile leak if cyst is connected to any biliary canaliculie.
