Vestibular schwannoma.
It's a benign tumor. Patient presents with hearing loss, difficulty in walking, facial deviation, giddiness, headache. Difficulty in swallowing and speech in advanced cases.
This man is a case of CP angle vestibular schwannoma operated 3 yrs back at some other hospital where only partial removal was done.
Patient presented with above mentioned complaints.
These tumors are dangerous and difficult to operate bcz of its area of origin. tumor is almost adhered to brain stem that carries all vital nerves necessary to life and function.
So risk of life and neurological deficit is high. So removal of such tumors require high degree of surgical expertise and experience.
Patient was operated at paras hospital successfully. Complete tumor removed.

Sensations(pain, touch temperature) from face on each side is carried to our brain by a nerve called TRIGEMINAL NERVE.
Irritation of this nerve due to Certain abnormal blood vessel compression, tumor etc can cause severe electric shock like sensation with debilitating facial pain. There can be some specific triggers that can start this pain as shown in pic. Many a times it mimics toothache and hence land up in multiple tooth extraction if not seen by a qualified doctor well in time. This pain is usually not relieved in spite of multiple medications, and its near death experience for such patients.
Recently operated One such patient who presented with severe facial pain not relieved by medication. Clinical history was suggestive of trigeminal neuralgia. There was a blood vessel compressing the trigeminal nerve at the site of its origin( REZ) from brain stem ( Pons).
Removal of this compression provides best possible outcome. After explaining all risk and benefit of other available options patient opted for surgical intervention.
Vascular compression was removed. Patient recovered fully. Now he is asymptomatic and not on any medicine. Microvascular Decompression of trigeminal nerve provides best outcome in such cases.
Neuro vascular surgery is advanced branch in neurosurgery being done at selected centers now available at my center.

Most severe type of brain hemorrhage occurs when wall of large blood vessels supplying brain gets thinned and finally ruptures. It's a death sentence unless appropriate measures are taken well in time.
Such thinning and protrusion of a part of wall of blood vessel is called ANEURYSM.
There are various modalities of management. Clipping ( open operation) and coiling (closed procedure). Each have it's own indication pros and cons.
Recently clipped a case of posterior communicating segment aneurysm in a young lady who presented with progressive drooping of right eyelids for one months and severe headache for a day.
CT SCAN SHOWED brain hemorrhage. Ct angioplasty of brain was done to look at blood vessels of brain. Bleed was from an aneurysm which was clipped and also brain hemorrhage was removed in same operation. Post op patient was discharged in a week.
