گردے اللہ تعالیٰ کی عطا کردہ ایک عظیم نعمت ہیں جو انسانی جسم میں انتہائی اہم کردار ادا کرتے ہیں۔ یہ خون کو صاف کرتے ہیں، جسم سے فاضل مادوں، زہریلے اجزاء اور اضافی پانی کو پیشاب کے ذریعے خارج کرتے ہیں، بلڈ پریشر کو متوازن رکھنے میں مدد دیتے ہیں، خون کے سرخ خلیات بنانے والے ہارمون کی تیاری میں حصہ لیتے ہیں اور جسم میں نمکیات، معدنیات اور تیزابیت کے توازن کو برقرار رکھتے ہیں۔ جب گردے کسی بیماری، انفیکشن یا دیگر وجوہات کی بنا پر اپنی صلاحیت کھونے لگتے ہیں تو جسم میں زہریلے مادے جمع ہونا شروع ہو جاتے ہیں، جس کے نتیجے میں مختلف پیچیدگیاں پیدا ہوتی ہیں۔ اس کیفیت کو گردوں کی بیماری یا Kidney Disease کہا جاتا ہے۔ اگر اس کی بروقت تشخیص اور مناسب علاج نہ کیا جائے تو یہ بیماری دائمی گردوں کی ناکامی (Chronic Kidney Disease) یا مکمل گردوں کی ناکامی (Kidney Failure) تک پہنچ سکتی ہے، جہاں مریض کو ڈائیلسز یا گردے کی پیوند کاری کی ضرورت پیش آتی ہے۔

گردوں کی بیماری کی سب سے عام وجوہات میں ذیابیطس (شوگر)، ہائی بلڈ پریشر، گردوں میں بار بار انفیکشن، پتھری، موروثی بیماریاں، درد کش ادویات کا بے جا استعمال، پانی کی شدید کمی اور بعض خودکار مدافعتی بیماریاں شامل ہیں۔ خاص طور پر شوگر اور ہائی بلڈ پریشر اگر کئی سال تک قابو میں نہ رہیں تو یہ آہستہ آہستہ گردوں کو نقصان پہنچاتے ہیں۔ اسی لیے ان بیماریوں میں مبتلا افراد کو وقتاً فوقتاً گردوں کے ٹیسٹ کرواتے رہنا چاہیے تاکہ کسی بھی خرابی کا بروقت پتا چل سکے۔

گردوں کی بیماری کی علامات شروع میں بہت ہلکی ہوتی ہیں، اسی وجہ سے اکثر مریض اس وقت ڈاکٹر کے پاس پہنچتے ہیں جب بیماری کافی بڑھ چکی ہوتی ہے۔ عام علامات میں چہرے، ہاتھوں اور ٹانگوں پر سوجن، پیشاب کی مقدار میں کمی یا زیادتی، پیشاب میں خون یا جھاگ آنا، جسم میں شدید کمزوری، ہر وقت تھکاوٹ، بھوک کی کمی، متلی، الٹی، جلد پر خارش، سانس پھولنا، نیند کی خرابی اور بلڈ پریشر کا مسلسل بلند رہنا شامل ہیں۔ اگر ایسی علامات محسوس ہوں تو فوراً کسی ماہر امراضِ گردہ (Nephrologist) سے رجوع کرنا چاہیے۔

جب گردے اس حد تک خراب ہو جائیں کہ وہ خون سے زہریلے مادے اور اضافی پانی خارج نہ کر سکیں تو مریض کی جان بچانے کے لیے ڈائیلسز کیا جاتا ہے۔ ڈائیلسز ایک ایسا طبی عمل ہے جس کے ذریعے مصنوعی طریقے سے خون کو صاف کیا جاتا ہے۔ اس کی دو بنیادی اقسام ہیں: ہیمو ڈائیلسز اور پیریٹونیل ڈائیلسز۔ ہیمو ڈائیلسز میں مریض کے خون کو ایک مشین کے ذریعے جسم سے نکال کر صاف کیا جاتا ہے اور پھر دوبارہ جسم میں واپس پہنچایا جاتا ہے۔ یہ عمل عام طور پر ہفتے میں دو سے تین مرتبہ کیا جاتا ہے اور ہر سیشن تقریباً چار گھنٹے جاری رہتا ہے۔ دوسری طرف پیریٹونیل ڈائیلسز میں مریض کے پیٹ کی اندرونی جھلی کو قدرتی فلٹر کے طور پر استعمال کیا جاتا ہے اور یہ طریقہ بعض مریض ڈاکٹر کی نگرانی میں گھر پر بھی انجام دے سکتے ہیں۔

ڈائیلسز کروانے والے مریضوں کو اپنی روزمرہ زندگی میں کئی احتیاطیں کرنا ضروری ہوتی ہیں۔ انہیں پانی اور نمک کی مقدار ڈاکٹر کی ہدایت کے مطابق محدود رکھنی چاہیے، پوٹاشیم اور فاسفورس والی غذاؤں جیسے کیلا، خشک میوہ جات، کولڈ ڈرنکس اور بعض دیگر اشیاء کے استعمال میں احتیاط برتنی چاہیے، شوگر اور بلڈ پریشر کو قابو میں رکھنا چاہیے اور ڈائیلسز کے لیے بنائے گئے فسٹولا یا کیتھیٹر کی صفائی کا خاص خیال رکھنا چاہیے تاکہ انفیکشن سے بچا جا سکے۔ اگر بخار، شدید درد، سوجن یا سانس لینے میں دشواری محسوس ہو تو فوراً ڈاکٹر سے رابطہ کرنا چاہیے۔

اگر مریض گردے کی پیوند کاری کے لیے موزوں ہو تو یہ علاج کا بہترین اور مؤثر طریقہ سمجھا جاتا ہے۔ گردے کی پیوند کاری میں ایک صحت مند گردہ مریض کے جسم میں منتقل کیا جاتا ہے۔ عطیہ دینے والا زندہ رشتہ دار بھی ہو سکتا ہے یا قوانین کے مطابق وفات یافتہ ڈونر بھی۔ کامیاب ٹرانسپلانٹ کے بعد بہت سے مریض معمول کی زندگی گزار سکتے ہیں، ملازمت کر سکتے ہیں، سفر کر سکتے ہیں اور اپنی روزمرہ سرگرمیاں بہتر انداز میں انجام دے سکتے ہیں۔

تاہم گردے کی پیوند کاری کے بعد سب سے اہم مرحلہ احتیاط اور مسلسل دیکھ بھال ہے۔ مریض کو عمر بھر وہ ادویات استعمال کرنا پڑتی ہیں جو جسم کو نئے گردے کو مسترد کرنے سے روکتی ہیں۔ ان ادویات کا ایک بھی ڈوز چھوڑنا خطرناک ثابت ہو سکتا ہے۔ مریض کو باقاعدگی سے خون اور پیشاب کے ٹیسٹ کروانے چاہئیں، ڈاکٹر کی مقرر کردہ تمام فالو اپ ملاقاتوں پر جانا چاہیے، صفائی کا خاص خیال رکھنا چاہیے، ہجوم اور متعدی بیماریوں سے حتیٰ الامکان بچنا چاہیے، متوازن غذا استعمال کرنی چاہیے، تمباکو نوشی اور نشہ آور اشیاء سے مکمل پرہیز کرنا چاہیے، مناسب ورزش کرنی چاہیے اور اپنے وزن کو قابو میں رکھنا چاہیے۔ اگر بخار، پیشاب میں کمی، جسم میں سوجن، شدید درد یا کسی بھی غیر معمولی علامت کا سامنا ہو تو فوراً اپنے معالج سے رابطہ کرنا چاہیے۔

یہ بات ہمیشہ یاد رکھنی چاہیے کہ گردوں کی بیماری سے بچاؤ علاج سے بہتر ہے۔ روزانہ مناسب مقدار میں پانی پینا، متوازن غذا کھانا، باقاعدہ ورزش کرنا، غیر ضروری درد کش ادویات سے پرہیز کرنا، شوگر اور بلڈ پریشر کو قابو میں رکھنا اور وقتاً فوقتاً طبی معائنہ کروانا گردوں کو صحت مند رکھنے کے بنیادی اصول ہیں۔ اگر کسی شخص کو شوگر، ہائی بلڈ پریشر یا گردوں کی بیماری کی خاندانی تاریخ ہو تو اسے خصوصی احتیاط اور باقاعدہ ٹیسٹ کروانے چاہئیں۔

Kidney Disease, Dialysis, and Kidney Transplant: A Complete Guide

The kidneys are among the greatest blessings bestowed upon us. They play a vital role in maintaining overall health by filtering waste products and toxins from the blood, removing excess water through urine, regulating blood pressure, producing hormones that stimulate red blood cell formation, and maintaining the body's balance of electrolytes, minerals, and acids. When the kidneys become damaged due to disease or injury, they gradually lose their ability to perform these essential functions. This condition is known as Kidney Disease. If left untreated, it can progress to Chronic Kidney Disease (CKD) or even Kidney Failure, where dialysis or a kidney transplant becomes necessary to sustain life.

The most common causes of kidney disease include diabetes, high blood pressure, recurrent kidney infections, kidney stones, inherited kidney disorders, excessive use of painkillers, severe dehydration, and certain autoimmune diseases. Diabetes and hypertension are the leading causes worldwide, as uncontrolled blood sugar and high blood pressure slowly damage the delicate blood vessels inside the kidneys. People living with these conditions should undergo regular kidney function tests to detect any abnormalities at an early stage.

Kidney disease often develops silently, with few or no symptoms during its early stages. As the disease progresses, patients may experience swelling of the face, hands, or legs, changes in urination, foamy or bloody urine, persistent fatigue, weakness, loss of appetite, nausea, vomiting, itchy skin, shortness of breath, sleep disturbances, and uncontrolled high blood pressure. Anyone experiencing these symptoms should consult a nephrologist (kidney specialist) as soon as possible for proper evaluation and treatment.

When the kidneys can no longer effectively remove waste products and excess fluid from the body, dialysis becomes necessary. Dialysis is a life-saving medical procedure that performs the filtration function of damaged kidneys. There are two main types of dialysis: Hemodialysis and Peritoneal Dialysis. During hemodialysis, blood is removed from the body, filtered through a dialysis machine, and then returned to the bloodstream. This treatment is usually performed two to three times a week, with each session lasting approximately four hours. Peritoneal dialysis, on the other hand, uses the lining of the abdomen as a natural filter and can often be performed at home under medical supervision.

Patients undergoing dialysis must carefully follow their doctor's recommendations. They should limit their intake of salt and fluids as advised, avoid foods high in potassium and phosphorus when recommended, maintain healthy blood sugar and blood pressure levels, and take proper care of their dialysis access site to prevent infections. Any signs of fever, swelling, severe pain, breathing difficulties, or infection should be reported to a healthcare provider immediately.

For many patients with end-stage kidney disease, a kidney transplant offers the best opportunity for a longer and healthier life. A kidney transplant involves surgically placing a healthy kidney from a living or deceased donor into the patient's body. Following a successful transplant, many individuals can return to normal daily activities, work, travel, and enjoy a significantly improved quality of life.

However, life after a kidney transplant requires lifelong commitment and medical care. Transplant recipients must take immunosuppressive medications every day to prevent their immune system from rejecting the new kidney. Missing even a single dose can increase the risk of rejection. Regular medical checkups, blood tests, and urine tests are essential to monitor kidney function. Patients should maintain excellent personal hygiene, avoid exposure to infections, eat a balanced diet, quit smoking, avoid alcohol and recreational drugs, exercise regularly, maintain a healthy body weight, and follow all medical advice. Any warning signs such as fever, reduced urine output, swelling, or pain should be reported to the transplant team without delay.

It is important to remember that prevention is always better than treatment. Drinking adequate water, eating a healthy and balanced diet, exercising regularly, avoiding unnecessary painkillers, controlling diabetes and high blood pressure, and undergoing routine health screenings can significantly reduce the risk of kidney disease. Individuals with diabetes, hypertension, or a family history of kidney disease should be especially vigilant and have their kidney function checked regularly.

Modern medicine has made tremendous progress in the treatment of kidney disease, offering hope to millions of patients around the world. With early diagnosis, proper medical care, adherence to treatment, a healthy lifestyle, and the support of family members, people living with kidney disease can lead productive and fulfilling lives.
