"Fever since yesterday. Should we do a dengue test today?"

Our physicians hear this question often this month. Pune city recorded 1,643 confirmed dengue cases from June to September, and September was the highest month. The honest answer is that the day of fever decides which test makes sense.

Here is what happens behind the test:
1. First 5 days of fever: NS1 antigen test. It looks for a protein of the virus itself.
2. From about day 5: IgM antibody test. It looks for your body's response, so it can be negative if done too early.
3. At every visit: CBC, for the platelet count and haematocrit. One report is a snapshot. The doctor watches the trend.

Something most families get wrong: the danger is not over when the fever comes down. That is the window when warning signs appear. Severe stomach pain, repeated vomiting, bleeding gums or nose, or unusual weakness means hospital the same day.

At home: paracetamol and fluids. No ibuprofen or aspirin unless your doctor says so.

Every festive season we see it in our dialysis unit in Chinchwad: new bangles, a watch or a fitted sleeve, all on the fistula arm. It looks lovely. We still ask you to move them to the other hand, and here is why.

Your AV fistula is your lifeline for dialysis. Bangles, a kada, a watch, a tight blouse sleeve, a heavy puja thali, even sleeping on the arm: anything that squeezes it can slow the blood flow, and a fistula that clots may stop working.

So on the fistula arm, please: no bangles, watch or tight thread, no tight sleeves, no BP check, injection or blood test, and no heavy bags.

Every morning, place your fingers on the fistula and feel for the buzz, called the thrill. It should feel the same every day. If it is weak or missing, or the arm is red, warm, swollen or painful, call the dialysis unit the same day.

If the fistula ever bleeds, press firmly with a clean cloth and raise the arm. If it has not stopped in 10 minutes, go to emergency.

Is someone in your family fasting for Navratri with diabetes or BP? Please read this before day two.

Three things can go wrong. Long gaps plus the usual medicines can drop sugar too low. Fried and sweet fasting foods can push it high. And too little water in this 35°C heat can cause dehydration and low BP.

What our physicians advise:
1. Ask before you fast. Your doctor may change the timing or dose of your tablets or insulin for the nine days. Do not stop or adjust medicines on your own.
2. Eat something every 3 to 4 hours. Add protein: curd, paneer, milk, nuts. Prefer fruit, rajgira and samak rice over fried sabudana and potato. Sendha namak is still salt.
3. Sip water, buttermilk or lemon water through the day. Kidney and heart patients should ask about fluid limits.
4. Check sugar more often than usual.

Break the fast at once if sugar is below 70 or above 300 mg/dL, or there is sweating, shaking, dizziness, confusion or vomiting. Chest pain or breathlessness is an emergency.

Pune is at 33 to 34°C this October and everyone is being told to drink more water. In our dialysis unit, patients are asking a fair question: "Does that apply to me too?"

For most people on haemodialysis, it doesn't. Whatever the weather, extra fluid stays in the body until the next session. It shows up as swollen feet, high BP and breathlessness, and it makes dialysis harder.

What helps more than water is less salt. Pickle, papad, farsan and packaged food are what make you thirsty. Cut those, and the limit gets easier.

A few tricks from our patients: use a small cup, suck an ice chip slowly and count it, rinse your mouth without swallowing, and stay out of the midday sun. Go easy on coconut water and fruit juice. They count as fluid and are high in potassium.

Call your unit if your weight jumps between sessions or you are breathless lying down.

Garba starts tomorrow, and Pune is still in its October heat.

Every Navratri our emergency team at Chetna Hospital, Chinchwad, treats people who felt dizzy or fainted on the garba ground. Most of them were healthy. They had danced for hours, fasted through the day and had hardly any water.

If you feel faint: stop, sit or lie down, raise your legs, move somewhere cool and airy, and sip water or ORS slowly. Do not try to finish the round.

If a friend collapses: lay them flat, raise the legs, clear the crowd and cool the skin with a wet cloth. Give nothing by mouth until they are fully awake.

Go to emergency if the person is not better in 30 minutes, is confused, has very hot dry skin, has a fit, or does not wake within a minute. Chest pain with fainting always needs emergency care.

If you are fasting, have fluids and a light meal before you leave home, within what your fast allows, and keep an ORS sachet in your bag.
