India's Total Fertility Rate (TFR) has fallen below the replacement level of 2.1. A replacement fertility rate of 2.1 means each generation produces enough children to replace itself. When fertility remains below this level for decades:
Fewer children are born.
The working-age population eventually shrinks.
The proportion of elderly people increases.
Population growth slows and can eventually decline.
Impact on Aging Population
As birth rates fall and life expectancy rises:
👴 More senior citizens require healthcare, assisted living, and social support.
👨⚕️ Increased burden on healthcare systems due to chronic diseases such as diabetes, hypertension, dementia, and cancer.
👨👩👧 Fewer young adults are available to support aging parents.
📉 Dependency ratio rises — fewer workers support more retirees.
Countries like Japan, South Korea, and Italy are already facing these challenges.
India currently enjoys a "demographic dividend" because of its young population. However, if fertility remains low for a prolonged period, India could gradually transition toward an aging economy.
Why Early Fertility Checks Become Important
Although population fertility is declining, infertility rates among couples are increasing due to delayed marriage, delayed childbearing, lifestyle disorders, obesity, PCOS, endometriosis, poor s***m quality, stress, and environmental factors.
Many couples assume pregnancy will happen naturally whenever they decide to start a family. Unfortunately, fertility declines ignificantly with age, especially after 30–35 years in women.
The New Public Health Message
Just as we encourage preventive health checkups for diabetes and heart disease, fertility assessment should become part of preventive reproductive healthcare.
