An interesting diagnostic pitfall in a female patient referred for ultrasound-guided biopsy, followed by CECT, after a prior FNAC reported malignant spindle cells.

During the biopsy, I noticed the lesion did not have the typical appearance of a metastatic lymph node. On CT, it was well-circumscribed and sharply delineated. Review of the PET/CT showed marked FDG avidity; however, no primary malignancy was identified.

Considering the imaging morphology, absence of another primary tumour and confirmatory histopathology, the final diagnosis was schwannoma.

Never skip the GE junction. 🔍

A colleague taught me this during residency, and it has helped me detect several incidental cancers.

This patient was referred for abnormal uterine bleeding. Imaging revealed an incidental GE junction lesion, with endoscopy and histopathology confirming squamous cell carcinoma.

The indication guides the scan—it should never limit the search.

As I step into my second year as Secretary of the IRIA (Indian Radiological and Imaging Association) Meghalaya State Chapter, I feel grateful for the opportunity to contribute to our growing radiology community.

At our Annual State Chapter Conference in collaboration with AI-Powered Medical Imaging Medikabazaar in Shillong last week, I had the privilege of serving as Quizmaster and delivering a faculty lecture on “FDG PET/CT in Oncology: What a Radiologist Must Know.”

Through the years Ive realized that teaching young radiologists is something I truly enjoy, and every such opportunity reminds me that learning never stops.

While protocols are important, this case demonstrates that active radiologist engagement at the scanner remains invaluable. A straightforward FOV adjustment made during the scan was a game-changer!
What’s the diagnosis?
