"Laparoscopic Cholecystectomy in Acute Cholecystitis"

Early laparoscopic cholecystectomy is the standard treatment for Acute Cholecystitis, ideally performed within 72 hours of symptom onset.

Inflammation, edema, and adhesions around the gallbladder make the procedure technically demanding. The key principle is achieving the Critical View of Safety before clipping the cystic duct and artery to prevent bile duct injury.

This surgery should be performed by a trained laparoscopic general surgeon, preferably with good experience in difficult gallbladder cases. When anatomy is unclear or inflammation is severe, conversion to open surgery is a safe and wise decision.

Minimally invasive approach offers less postoperative pain, shorter hospital stay, and faster recovery, even in acute cases when performed with proper expertise.

"Management Of Large Direct Pseudosacs In Laparoscopic Inguinal Hernia Surgery"

Laparoscopic Inguinal Hernia repair is a safe and effective alternative to open inguinal hernia surgery with benefits like less pain and faster recovery, especially for bilateral hernias or in patients with a history of previous hernias, but its success is highly dependent on the surgeon's expertise and can be less reliable in less experienced hands.
In case of large direct hernias, Pseudosacs should always be managed either by plication or fixation to coopers ligament so as to obliterate the space that in turn prevents mesh migration in large Pseudosacs, reduces or prevent seroma formation, alleviates patient anxiety by minimizing the inguinal bulge in post operative period.
