I think about this every time I take on a complex shoulder or spine case. The protocol is the easy part. Actually listening to why someone’s guarding, why they’re scared to lift their arm or bend their spine,that’s where the real work starts.
Marco Pierre White once said the difference between a laborer, a craftsman, and an artist is the heart. Rehab is no different. Anyone can give you exercises. Recovery happens when someone actually gives a damn.

“If I got non-specific back pain tomorrow, here’s my 3-step plan and yes, I’m apologising in advance for how unscientific this sounds. 😅”

Disclaimer: this is not a systematic review. This is not a randomized controlled trial. This is one physiotherapist’s spine, tested on exactly one subject (me), n=1, no control group, definitely no ethics approval. Proceed accordingly. 😅

1. I would NOT rest.
Bed rest for acute back pain is basically the medical equivalent of “have you tried turning it off and on again” except it doesn’t even work. Movement is medicine here. Gentle, pain-guided movement keeps things from stiffening up and spiraling into “why did I let this become a whole Situation.”
2. Hot pack. Non-negotiable.
Is there a mountain of high-grade evidence proving heat therapy changes long-term outcomes? Not really. Do I not care? Also not really. It feels good, it relaxes guarded muscles, and sometimes “this makes me feel human again” is a legitimate clinical outcome. Personal bias, fully disclosed.

3. I would sleep. Properly.
Pain and poor sleep are in a toxic relationship with each other, and they both make everything worse. Prioritizing real rest (not bed rest, actual sleep) helps your nervous system calm down and stop treating your back like a five-alarm fire.

None of this replaces an actual assessment if your pain is severe, doesn’t improve, or comes with red flags (numbness, weakness, bowel/bladder changes see a professional immediately, don’t @ me). But for the everyday “I slept weird” or “gym got the better of me” back pain? This is my honest, slightly biased, very human approach.

Schroth for scoliosis, plain and simple
Think of scoliosis as your spine twisting and curving like a garden hose with kinks. Schroth is the method that unkinks it, but only if you plan every move.
It’s not random stretching. It’s like giving your spine step-by-step directions to stand taller. That’s why you need a trained Schroth therapist to set it up. Guessing can make the curve worse.

You know the feeling when you wake up with a stiff neck or when you are hunched over a screen all day, upper back screaming, shoulders practically touching your ears?

This one move targets thoracic rotation and lateral flexion the movements your spine craves but rarely gets in daily life. Think of it as wringing out tension you didn’t know you were storing.

✅ Safe practice tips:

• Stop if you feel sharp pain, not just stretch discomfort
• Never force rotation — this isn’t a competition
• Mild tightness = good. Pain = your cue to stop.
• If you’re unsure, consult a physiotherapist before adding this to your routine
