✨ Immediate Loading Full-Arch Rehabilitation – Lower Jaw Implants with Hybrid Denture ✨
A patient presented with edentulous lower arch, difficulty in chewing, and an upcoming family wedding within a week. The patient desired a fixed solution that would restore function and appearance quickly.
After detailed clinical and radiographic evaluation, strategic dental implants were placed in the lower arch and connected using multi-unit abutments. An immediate loading protocol was followed, allowing the patient to receive a fixed hybrid prosthesis shortly after implant placement.
The upper arch was also evaluated, and implant-supported rehabilitation was recommended to achieve optimal long-term function and occlusal harmony. However, the patient chose not to proceed with upper arch implant treatment at this stage.
Considering the patient's timeline and desire to attend the wedding with confidence, a customized lower hybrid denture was fabricated and delivered under immediate loading principles.

A challenging deep bite rehabilitation case involving severely compromised lower anterior teeth with advanced periodontal breakdown, mobility, infection, and unhealthy granulation tissue.

The excessive deep bite had contributed to traumatic loading on the lower anterior teeth, further worsening mobility and bone loss over time.

Treatment performed:

Patient reported to the clinic with complaints of pain and discomfort around the TM joint region.
Signs and symptoms included:
• Pain near the preauricular area aggravated during chewing and mouth opening
• Clicking and popping sounds from the TM joint
• Morning jaw stiffness and muscle fatigue
• Tenderness over muscles of mastication
• Difficulty in wide mouth opening
• Deviation during opening and closing movements
• Occasional headache and facial pain radiating towards temporal region
• Clenching and parafunctional habits history
On clinical examination, occlusal discrepancies and muscle tenderness were noted. A permissive splint therapy was planned to achieve neuromuscular relaxation and joint decompression.
Patient showed significant symptomatic relief with reduction in pain, improved mouth opening, muscle relaxation, and better mandibular movements after splint therapy.
Permissive splints remain an effective conservative approach in management of TMJ dysfunction and occluso-muscular disorders.
