📢 Announcement – IAPMR Fellowship Programs 2026–27

Dear Colleagues,
The Indian Association of Physical Medicine and Rehabilitation (IAPMR) is pleased to announce the commencement of admissions for the IAPMR Fellowship Programs for the Academic Year 2026–27.

The following one-year full-time fellowship programs are being offered at IAPMR-accredited centres across India:

🔹 *Fellowship in Neuro-Rehabilitation*
Focused on advanced training in the rehabilitation of patients with stroke, spinal cord injury, traumatic brain injury, cerebral palsy, neurodegenerative disorders, and other neurological conditions through a multidisciplinary and evidence-based approach.

🔹 *Fellowship in Pain Medicine*
Designed to provide comprehensive training in the assessment and management of acute and chronic pain conditions, including hands-on exposure to interventional pain procedures, ultrasound-guided techniques, and multidisciplinary pain rehabilitation.

These fellowships aim to develop highly skilled rehabilitation specialists with advanced clinical, procedural, academic, and research competencies.

As this marks the first year of the IAPMR Fellowship Program, the Association is highly optimistic and enthusiastic about its success. We believe these fellowships will contribute significantly to the growth of subspecialty training and strengthen rehabilitation services across the country.

We PMR Physicians forms an integral part in haemophilia management and care.

Pain Management in Hemophilia – PMR Perspective
🔴 1. Types of Pain in Hemophilia
Acute Pain (Bleeding episodes)
Hemarthrosis (knee, ankle, elbow most common)
Muscle bleeds (iliopsoas → femoral neuropathy risk)

👉 Features: sudden onset, warmth, swelling, ↓ROM
Chronic Pain (Hemophilic arthropathy)
Recurrent bleeds → synovial hypertrophy → cartilage destruction
Leads to:
Contractures
Muscle wasting
Malalignment
👉 Similar to secondary osteoarthritis but earlier + more aggressive

💊 2. Pharmacological Management
Acute Phase (Bleed-related pain)
Factor replacement (definitive treatment)
Paracetamol (first-line)
Short course opioids if severe
⚠️ Avoid:
NSAIDs (platelet dysfunction, bleeding risk)
IM injections
👉 Selective COX-2 inhibitors (e.g., Celecoxib) may be used cautiously
Chronic Pain
Paracetamol ± COX-2 inhibitors
Duloxetine / amitriptyline (if central/neuropathic component)
Avoid long-term opioids

3. Rehabilitation

During Acute Bleed
POLICE principle (not aggressive RICE)
Protection
Optimal loading (after factor replacement)
Ice
Compression
Elevation
👉 Immobilization short-term only
Post-Acute Phase
Early ROM exercises
Isometric → isotonic strengthening
Proprioception training
Gait training
👉 Prevents chronic arthropathy progression
Chronic Stage
Muscle strengthening (quadriceps, calf)
Joint protection strategies
Orthotics (ankle braces)
Assistive devices if needed
Interventions like casting for joint protection/intramuscular haematoma prevention
Traction therapy in the management of flexion contractures
Intra-articular interventions (with factor cover)
Steroid injections
Radiosynovectomy (Yttrium-90)
👉 Indicated in chronic synovitis with recurrent bleeds

Physical Medicine and Rehabilitation. PM&R. Physiatry. Rehabilitation Medicine.
Different names, one powerful mission: restoring function.

In medicine, we often focus on survival.
But in Physical Medicine & Rehabilitation, we ask a different question:

👉 “Now that you’ve survived… how do you live better?”

Whether it’s:
• Walking again after a spinal cord injury
• Speaking and eating after a stroke
• Being able to remember things after traumatic brain injury
• Returning to school with cerebral palsy

PMR is about rebuilding lives, not just repairing bodies. We are the doctors who speak the language of function by bridging the gap between treatment and real-world living. Because recovery doesn’t end at acute care discharge. That’s where rehabilitation begins.
