Complex Angioplasty for a Severely Calcified LAD

Not every coronary blockage can be treated by simply placing a stent.

In this case, severe calcium in the LAD required careful lesion assessment and preparation before stenting.

IVUS → Calcium assessment → Calcium modification → ROTA → Cutting Balloon → Stent → Final IVUS

IVUS helped us understand the calcium before treatment and assess the final stent result after angioplasty.

Complex coronary intervention is about understanding the lesion, choosing the right technology and optimising the final result.

A wonderful academic experience at iPCI 2026 – Where PCI Meets Precision.

Privileged to serve as a judge for two complex coronary case-presentation sessions covering:

Recanalization, Physiology & Angiographic Ambiguity
and
ISR, Thrombosis & Stent Failure.

It was an engaging morning of discussions around complex angioplasty, CTO PCI, IVUS, OCT, coronary physiology, restenosis and stent failure.

The biggest value of intracoronary imaging and physiology is that they help us move beyond what the angiogram appears to show — towards understanding why a lesion behaves the way it does and how best to treat it.

Great to meet colleagues, exchange ideas and learn from distinguished faculty at iPCI 2026.

A cardiac arrest survivor. A rare double myocardial infarction. Two culprit coronary arteries.

This patient survived a cardiac arrest and was found to have a rare simultaneous anterior and inferior wall myocardial infarction.

Echocardiography showed severely reduced heart-pumping function with an EF of approximately 20%.

Coronary angiography identified culprit lesions involving both:

🔴 RCA — Right Coronary Artery
🔴 LAD — Left Anterior Descending Artery

Emergency PTCA was performed to both culprit coronary vessels.

This case highlights why immediate CPR, early recognition of a heart attack, rapid coronary angiography and timely angioplasty can be life-saving.
