🩺 Every mL Counts: The Critical Role of the Cath Lab Technologist in Preventing Contrast-Induced Nephropathy (CIN)
Contrast-Induced Nephropathy (CIN), now more commonly referred to as Contrast-Associated Acute Kidney Injury (CA-AKI), remains one of the most preventable complications following coronary angiography and PCI. While physicians determine the treatment strategy, Cath Lab Technologists play a vital role in ensuring that contrast is used safely, efficiently, and responsibly throughout the procedure.
A vigilant technologist can make a measurable difference by:
✅ Reviewing baseline renal function (Serum Creatinine/eGFR) before the procedure.
✅ Calculating and communicating the Maximum Allowable Contrast Dose (MACD) before contrast injection begins.
✅ Preparing injectors and manifolds to avoid unnecessary contrast wastage.
✅ Tracking cumulative contrast volume in real time and informing the operator at predefined thresholds.
✅ Encouraging optimal imaging techniques that reduce repeat angiographic runs.
✅ Avoiding unnecessary test injections and duplicate cine acquisitions.
✅ Maintaining accurate documentation of total contrast volume in the procedure record.
✅ Supporting hydration protocols and departmental quality initiatives aimed at reducing CA-AKI.
Preventing kidney injury is not the responsibility of one individual.. it is a team effort. Every member of the cath lab contributes to patient safety, and the technologist serves as the constant guardian of contrast management throughout the procedure.
