A patient came to the laboratory for a routine CBC test.
The automated analyzer reported low platelet count (thrombocytopenia) in the EDTA sample. Since the patient had no clinical signs of bleeding or bruising, we suspected EDTA-dependent platelet clumping (pseudothrombocytopenia).
A peripheral blood smear from the EDTA tube showed platelet aggregation, which can falsely lower the platelet count in automated analyzers.
To confirm, we repeated the test using a sodium citrate tube. The platelet count returned within the normal range, confirming that the low platelet result in EDTA was artificial, not true thrombocytopenia
