CBCT in Endodontics: What Changed from 2015 to 2025

The 2025 AAE/AAOMR position statement marks a clear shift from viewing CBCT as merely supplemental to recognizing it as a strategic diagnostic and treatment-planning tool when 2D imaging is inadequate. The focus moves from strict dose minimization to dose optimization, supporting task-based, limited field-of-view protocols tailored to diagnostic needs.

Greater emphasis is placed on image artifacts, especially from metal and root-filling materials, and their impact on diagnostic accuracy. Clinical indications are better defined and supported by stronger evidence, showing CBCT often changes diagnosis and treatment planning—particularly in retreatment, suspected vertical root fractures, resorption, trauma, and surgical cases.

The update also raises expectations for training and interpretive responsibility, including collaboration with oral and maxillofacial radiologists. Finally, CBCT is positioned as a foundation for AI-driven diagnostics and digitally guided endodontics.

Bottom line: CBCT is not for routine screening, but when used judiciously with optimized protocols and proper expertise, it is now central to precision-based endodontic care.
