🦷⚡ PET IS NOT JUST A DIFFERENT INSTRUMENT. IT’S A DIFFERENT APPROACH.

For decades, dental extractions have relied mainly on rotary drills, elevators, luxators and forceps.

They work.
But today we can approach extractions differently.

PET – Piezoelectric Extraction Technique changes the philosophy:

🐱 Atraumatic maxillary canine extraction with PET – Piezoelectric Extraction Technique

A complicated crown fracture doesn’t mean the extraction has to be traumatic.

In this case, I’m showing the extraction of a fractured maxillary canine in a cat using PET, integrated with conventional luxators, elevators and extraction forceps.

The goal is simple: use technology to reduce force and surgical trauma.

PET allows controlled piezoelectric action around the root, facilitating progressive luxation while preserving as much alveolar bone as possible.

🔹 Less force
🔹 Less bone removal
🔹 Less bleeding
🔹 Less postoperative edema
🔹 Less pain
🔹 Better healing
🔹 And, above all, much faster extractions

With the right technique and experience, even full-mouth extractions in cats can be completed in approximately 1 hour.

But PET is not simply “using piezosurgery for extractions.”

This technique is possible thanks to the world’s only veterinary-specific piezosurgery insert kits, developed through the collaboration between ESACROM and myself, specifically for veterinary dentistry and oral surgery.

Other piezosurgery inserts were originally designed for human dentistry and do not allow PET to be performed in the same way.

No rotary instruments. No force.

PET is a technique that we developed together with ESACROM specifically for Veterinary Dentistry, using dedicated piezoelectric inserts designed for dental and oral surgery in our patients.

But this case also carries an important educational message.

Unfortunately, almost every week I see similar cases involving persistent deciduous canines.

Often, simply because of limited specific training in veterinary dentistry, owners are advised to wait until the dog is one year old or even older before considering extraction.

In other cases, extraction is attempted without intraoral dental radiographs and the long, fragile deciduous root is fractured and left behind.

Even worse, owners are sometimes advised to give hard chewing objects such as antlers or bones, hoping that the deciduous tooth will eventually fall out.

This can produce exactly what we see in this patient: fracture of the crown with retention of the root beneath the gingiva.

The biological concept is actually very simple.

During normal eruption of the permanent tooth, the root of the corresponding deciduous tooth should undergo physiological root resorption.

If this process does not occur properly, the persistent deciduous tooth still has a developed root.

And a tooth with a developed root will not simply “fall out” because we wait longer or because the dog chews something hard.

It may remain in place — or it may fracture, leaving the root behind.

👉 Persistent deciduous teeth require proper diagnosis, intraoral dental radiographs and, when indicated, professional extraction with complete removal of the root.

🐶 Dopo tanti anni in canile, questo mix Pitbull ha finalmente trovato una famiglia. Purtroppo aveva riportato la frattura di entrambi i canini mascellari, ma la sua nuova proprietaria aveva un unico desiderio: salvargli i denti e permettergli di giocare di nuovo senza dolore.

In questo video mostro una tecnica che utilizzo per ottenere un accesso ideale alla terapia canalare direttamente dal sito di frattura mediante piezochirurgia. L’accesso viene ampliato e deterso in modo controllato, per poi essere strumentalizzato con Peeso reamers e lime endodontiche.

L’unità piezoelettrica Surgysonic Moto di consente inoltre l’attivazione ultrasonica dell’EDTA durante il trattamento endodontico.

🐶 After spending many years in a shelter, this Pit Bull mix finally found a loving home. Sadly, both maxillary canine teeth had been fractured, but his new owner had one goal: save them so he could finally enjoy playing again without pain.

In this video I demonstrate how piezosurgery can be used to create an ideal endodontic access directly through the fracture site. The access is enlarged and cleaned in a controlled manner before canal preparation with Peeso reamers and endodontic files.

The Surgysonic Moto piezo unit also allows ultrasonic activation of EDTA during root canal treatment.
