台湾・コロンビアからの手術見学
Welcome to TOYAMA from Taiwan and Colombia!

台湾、コロンビアの脳神経外科医が富山大学脳神経外科に手術見学に来てくれました。台湾 台中にあるTaichung Veterans General HospitalのDr. Chia-Ning Changは医師としても母としても素晴らしい女性で、6月17日〜7月10日、じっくりとわれわれの流儀を学んでくれました。3年前に富山を訪れたDr. Chung Hsin Leeも、彼女に随行して2日間再訪してくれました。コロンビア・ボゴタにあるSamaritan University HospitalのDr. Camilo Armando Benavides Burbanoは、陽気かつ大変真面目な医師で、6月30日〜7月10日、積極的にたくさんのことを学んでくれました。やはりDr. Leeの推薦で、台湾 台北にあるNational Taiwan University HospitalのDr. Chiu-Hao Hsuは台湾でバイパス手術を中心に活躍する優秀な医師で、7月1日〜7月25日、多くのことを学び、ディスカッションに花を咲かせてくれています。彼らが富山での学びを糧に、それぞれの国に戻ってからも脳神経外科医としてさらに成長して患者さんの健康のために尽くしてほしいと願っています。

Neurosurgeons from Taiwan and Colombia visited the Department of Neurosurgery at the University of Toyama to observe our surgical procedures and managements. Dr. Chia-Ning Chang from Taichung Veterans General Hospital in Taichung, Taiwan—an exceptional woman who excels as both a physician and a mother—spent the period from June 17 to July 10 thoroughly learning our surgical technique and opinion. Dr. Chung Hsin Lee, who had previously visited Toyama three years ago, accompanied her and returned for a two-day visit as well. Dr. Camilo Armando Benavides Burbano from Samaritan University Hospital in Bogotá, Colombia, is a cheerful yet deeply dedicated physician who actively engaged in learning from June 30 to July 10. Also visiting upon Dr. Lee’s recommendation is Dr. Chiu-Hao Hsu from National Taiwan University Hospital in Taipei; a highly skilled neurosurgeon specializing in bypass surgery, he has been learning a great deal and engaging in lively, fruitful discussions since his arrival on July 1. It is my sincere hope that they will use the knowledge gained in Toyama as a foundation for their continued growth as neurosurgeons and dedicate themselves to the health and well-being of their patients upon returning to their respective countries.

2012年6月、デンマークから一人の女の子が黒田の手術を受けるために富山大学附属病院まで来てくれました。富山大学附属病院では、初めての海外からの患者さんの受け入れでした。
この子はヨーロッパの病院でもやもや病と診断されて既にバイパス手術を受けていましたが、頻回の脱力発作は一向に治らず富山に来た時にはグッタリとした様子でした。女の子本人もご両親も遠い異国までやってきて、とても不安だったと思います。
黒田は2回に分けて左右大脳に対するrepeat bypassを実施しました。幸い、この子の脱力発作は消失してくれて、すっかり元気になってくれました。帰国する前に立山室堂まで一緒に遊びに行けるまでに回復してくれました。
その後も黒田はデンマークの主治医や両親との連絡を絶やさなかったほか、北欧に出張した際はコペンハーゲンで診察させてもらっていました。今月2026年6月、ついに彼女は元気に高校を卒業したと、先日お母さんから連絡がありました。
先達から教わった思想、伎倆を自分でも発展させて、それを国内の患者さんはもちろん、海外の患者さんにも役立てることができて大変嬉しく感じています。これからも彼女が素敵な女性として充実した人生を送れるよう、心より祈念しています。

In June 2012, a young girl traveled from Denmark to Toyama University Hospital to undergo surgery performed by Prof. Kuroda. This marked the first time the hospital had accepted a patient from overseas.
She had previously been diagnosed as moyamoya disease in Europe and had already undergone bypass surgery; however, she continued to suffer from frequent episodes of transient ischemic attack (TIA) and arrived in Toyama in a state of extreme exhaustion. Prof. Kuroda imagines both she and her parents felt a great deal of anxiety traveling to such a distant, foreign country.
Prof. Kuroda performed repeat bypass surgeries on both the left and right hemispheres of her brain in two separate procedures. Fortunately, her episodes of TIA ceased after surgeries, and she made a full recovery. She recovered enough to join us for a day trip to Mt. Tateyama Murodo before returning home.
Since then, Prof. Kuroda have maintained contact with her parents and her primary physician in Denmark, and he have even examined her in Copenhagen during his business trips to Europe. Just recently, her mother emailed him that she has successfully graduated from high school this month.
It brings him great happiness to have further developed the philosophies and surgical techniques passed down by his predecessors and to have applied them for the benefit of patients—not only in Japan but also from abroad. Prof. Kuroda sincerely hopes that she continues to lead a fulfilling life and grows into a wonderful woman.

BerlinのChariteチームを率いるProf. Peter Vajkoczyは、長年のよき友人です。昨年、彼のもとから研修医のDr. Robert Mertenが富山に研修に訪れました。その際のいくつものディスカッションの一つとして、もやもや病に対するバイパス手術の際に知悉しておくべき中硬膜動脈と頭蓋骨との解剖学的関係について、彼が素晴らしい解析をしてくれました。その結果が、6月5日、Journal of Neurosurgery誌に出版されたのでご報告させていただきます。

Professor Peter Vajkoczy, who leads the Berlin Charite team, is a long-time friend of mine. Last year, Dr. Robert Merten, a resident who are learning under the direction of Prof. Vajkocay, came to Toyama for several week-training. Based on one of our many discussions, he provided an excellent analysis of the anatomical relationship between the middle meningeal artery and the skull, which is crucial knowledge for bypass surgery for moyamoya disease. I am pleased to report that his findings were published in the Journal of Neurosurgery on June 5th.

After fontanelle closure, the likelihood of the MMA traversing a bony canal located within one thumb's width posterior to the pterion increases with age. Comprehensive knowledge of MMA anatomy is crucial for its preservation during bypass surgery.

Japanese Most Famous and Heaviest Textbook of Neurosurgery 14th. Edition

太田富雄先生の名著「脳神経外科学 第14版」(金芳堂)が発刊されました。 黒田は、今回も第5章「脳代謝、脳血流と脳虚血」(計27ページ)と第6章§9「虚血性脳血管障害」(計119ページ)を担当させていただきました。大変栄誉なことです。これで約13年にわたって第12版、第13版、第14版で執筆させていただきました。おそらく、これが最後だと思います。執筆中は苦しくも楽しい時間を過ごすことができました。関係者の皆さまに心より感謝いたします。

Professor Tomio Ota's renowned textbook, "Neurosurgery, 14th Edition" (Kinbundo), has been published. Prof. Kuroda was once again in charge of Chapter 5, "Brain Metabolism, Cerebral Blood Flow, and Cerebral Ischemia" (27 pages), and Chapter 6, Section 9, "Ischemic Cerebrovascular Disease" (119 pages). It is a great honor to be involved in this wonderful project. He has contributed to the 12th, 13th, and 14th editions over approximately 13 years. This will probably be his last. He had a challenging but enjoyable time while writing. He would like to express my sincere gratitude to everyone involved.

まもなく「脳ドックのガイドライン2026」を中外医学社から発刊します。今回は前版の「脳ドックのガイドライン2019」を7年ぶりに大幅に改訂して、最新の知見に基づいて脳卒中や認知症の予防についても触れています。今回、黒田は「脳ドックのガイドライン2026」改訂委員会の委員長として関係者の皆様と協力していただきながら、その職責を果たすことができました。どうもありがとうございました。
このガイドラインが、脳ドックに携わる皆様のみならず、無症候性の脳神経疾患を有する患者さんを診療する皆様にとって有益な書となれば望外の喜びです。

The “Brain Dock Guidelines 2026” will soon be published by Chugai Medical Publishing. This edition represents a major revision of the previous “Brain Dock Guidelines 2019” after seven years, incorporating the latest findings and addressing stroke and dementia prevention. As chair of the revision committee for the “Brain Dock Guidelines 2026,” Prof. Kuroda was able to fulfill my responsibilities with the cooperation of all involved parties. I am deeply grateful.
It would be our greatest pleasure if this guideline proves to be a valuable resource not only for those involved in brain dock examinations but also for all clinicians treating patients with asymptomatic neurological disorders.

当科の白石啓太朗先生の論文がScientific Report誌に掲載されました。
東北大学の出澤真理先生が発見した多能性幹細胞であるMuse細胞を、マウス脳挫傷モデルに経静脈的に移植したところ、コントロール群や骨髄間質細胞(BMSC)を移植した群よりも運動機能が有意に改善することが判明しました。多数のMuse細胞が脳挫傷の周辺に生着して神経細胞や乏突起神経膠細胞に分化していました。
出澤先生とはかれこれ15年以上共同研究を続けていますが、今後も引き続きさらなる研究成果を公表していきたいと思います。

Intravenous transplantation of multi-lineage differentiating stress enduring cell promotes functional recovery after traumatic brain injury in mice

Abstract
Multilineage-differentiating stress-enduring (Muse) cells are a unique subset of endogenous stem cells that are non-tumorigenic and possess pluripotent-like traits with macrophage-like functions. These cells are naturally present in bone marrow, peripheral blood, connective tissues of various organs, and the umbilical cord. Muse cells are defined by their expression of stage-specific embryonic antigen-3 (SSEA-3) and their high tolerance to cellular stress such as ischemia, oxidative injury, and inflammation. This study aimed to explore the therapeutic effects of intravenous administration of Muse cells on functional recovery after traumatic brain injury (TBI). Mouse TBI model was produced by applying a 3.5-mm-diameter copper cylinder cooled with liquid nitrogen to the right parietal bone. At 7 days after TBI, the animals randomly received an injection of either 6.0×104 human Muse cells (Muse group), 6.0×104 human mesenchymal stromal cells (low-dose MSC group), 1.6×106 human MSCs (high-dose MSC group), or PBS (vehicle group) through the tail vein (n=10 in each group). Motor function was assessed using a rotarod and cylinder test for 84 days after TBI. Subsequently, histological analysis was performed to explore the engraftment and phenotypic fate of Muse cells in the brain. Functional recovery was significantly enhanced on both rotarod and cylinder tests at 28 days and 42 days after TBI in Muse group, respectively, but not in vehicle group and low-dose MSCs group. Only limited effect was observed on cylinder test at 84 days after TBI in high-dose MSCs. Human mitochondria-positive cells were densely engrafted in the peri-TBI area in Muse group, but their number was very few in low- and high-dose MSC groups. About 60% and 20% of Muse cells were positive for NeuN and GSTpi, respectively. The findings strongly suggest that intravenously administered Muse cells aggressively migrate towards the injured brain, express specific markers for neurons and oligodendrocytes, and enhance the recovery of motor function in mice TBI model.

国際脳卒中会議2026@ニューオリンズ
International Stroke Conference 2026@ New Orleans

2026年2月4-6日、米国ニューオリンズで開催された国際脳卒中会議 (ISC2026)に参加してきました。今回はホノルルからのハシゴでしたが、思っていた以上に遠くて疲弊しました。
今回は、招待シンポジウム「もやもや病再考」のcreator + speakerとしての参加でした。国立循環器病研究センターの猪原匡史先生にもご登壇いただき、RNF213血管病について素晴らしいご講演を賜わりました。黒田はディベートセッション「間接バイパス vs 直接・複合バイパス」にて、前者を推すEdward Smith教授(ボストン)とともに、後者を推す立場から講演しました。
今回のシンポジウムでは、おそらく世界で初めて無症候性もやもや病への外科治療をめぐるディベートもありました。われわれAMOREグループが2012年から続けている研究の知見も多数引用されており、長年のメンバーの努力がようやく認められつつあることを実感できた旅でした。
フライトの関係でやむを得ずホノルルで一泊、これから帰国します。いつも留守を守ってくれている先生たちに感謝です。

Prof Kuroda participated in the International Stroke Conference (ISC2026) held in New Orleans, USA on February 4-6, 2026. This time I reached there from Honolulu, but it was farther than he thought and he was really exhausted.

This time, Prof Kuroda participated as a creator + speaker of the invited symposium "Moyamoya Disease Revisited". Dr. Masafumi Inohara of the National Cerebro- and Cardiovascular Center also took the stage and gave a wonderful lecture on RNF213 vascular disease. Kuroda gave a lecture on the debate session "Indirect Bypass vs Direct and Combined Bypass" with Professor Edward Smith from Boston, who recommends the former, and from the perspective of recommending the latter.

At this symposium, there was probably the first debate in the world about surgical treatment for asymptomatic moyamoya disease. Many of the findings that our AMORE group has been conducting since 2012 have also been cited, and it was a journey that made him realize that the long-standing efforts of his amazing members are finally being recognized.

Due to the flight schedule, he has no choice but to stay overnight in Honolulu, and now he will return home. He would like to thank his department members who are always taking care of thier patients without him.

われわれAMORE Groupからの新しい論文がCerebrovasc Dis誌に出版されました。
2012年から無症候性もやもや病の予後を調査中のAMORE研究にて、川崎医科大学脳神経外科の菱川朋人教授が新たな「AMOREスコア」を創案してくれました。このスコアは年齢、Grade-2 choroidal anastomosis、微小出血の3つだけの因子を用いた簡便なものですが、発見から5年間における脳出血のリスクをきわめて高い精度で予測できることが判明し、日常臨床でも簡単に応用可能なスコアです。
この論文は、昨年12月に受理されていましたが、迂闊にも本年1月30日にオンライン出版されていたことを知りませんでした。本日参加したInternational Stroke Conference 2026のシンポジウムにて、米国の先生がこの論文を引用して紹介してくれて、自分たちの論文の出版を初めて知るという体験は人生初でした😅

A new paper from our AMORE Group has been published in Cerebrovasc Dis.
In the ongoing AMORE study investigating the natural history of asymptomatic moyamoya disease since 2012, Professor Tomohito Hishikawa from Department of Neurosurgery, Kawasaki Medical School, developed a novel “AMORE Score.” This score is simple, using only three factors: age, Grade-2 choroidal anastomosis, and microbleeds. However, it can predict the five-year risk of cerebral hemorrhage with extremely high accuracy and is easily applicable in daily clinical practice.
This paper was accepted last December, but I carelessly didn't realize it had been published online on January 30th of this year. At the symposium I joined today at the International Stroke Conference 2026, the speaker from the US kindly cited and introduced this paper. It was the first time in my life that I got to know about the publication of our own paper through a citation by other researchers at a conference. 😅

The 5-year risk of hemorrhagic stroke in patients with asymptomatic MMD can be adequately estimated using the AMORE score.

大宅宗一先生(群馬大学)と清水暢裕先生(関東脳神経外科病院)にお声がけいただき、2026年2月1日から5日までホノルルにて開催されている第11回 Joint Neurosurgical Conventionに参加しました。富山大学脳神経外科からは、黒田のほか堀 恵美子先生が参加しました。黒田は「Surgical Technique and Perioperative Managements for Moyamoya Disease - TOYAMA Experience」と題して発表させていただきました。素晴らしい気候のもと、充実したプログラムで多くのことを学ばせていただきました。国内外の数多くの皆さんに感謝します。
これから、New Orleansで開催されるInternational Stroke Conferenceに参加するため飛びます。

I was invited by Professor Soichi Oya (Gunma University) and Professor Nobuhiro Shimizu (Kanto Neurosurgery Hospital) to participate in the 11th Joint Neurosurgical Convention held in Honolulu from February 1 to 5, 2026. From the Department of Neurosurgery at University of Toyama, I and Dr. Emiko Hori also attended the meeting. I presented on “Surgical Technique and Perioperative Managements for Moyamoya Disease - TOYAMA Experience.” Under wonderful weather conditions, the rich program allowed me to learn a great deal. I am grateful to the many people from Japan and abroad.
I am now flying to New Orleans to attend the International Stroke Conference.
