A 42-year-old male presents with a three-week history of severe, sharp, radiating pain in his right lower extremity.

The patient reports the sudden onset of lower back pain three weeks ago after lifting heavy boxes. Within 24 hours, the localized back pain subsided significantly, but a sharp, electric shock-like pain began radiating through his right buttock, down the side of the thigh, and into the calf and the foot.

The pain is currently rated an 8/10 on the visual analog scale (VAS). It is significantly worsened by sitting, coughing, or bending forward, and is partially relieved by standing or lying with a pillow under his knees.

He reports mild numbness along the lateral aspect of his right calf but denies any bowel or bladder incontinence, saddle anesthesia, or progressive lower extremity weakness.

One of the biggest misconceptions in sports medicine is that a successful operation automatically means a successful return to sport.

It doesn't.

A ligament can heal. A tendon can be repaired. An X-ray can look perfect.

But returning to sport demands much more than that.

Your strength needs to be restored. Your balance, coordination, and confidence need to return. Just as importantly, your body needs to relearn how to cope with the sudden changes of direction, acceleration, jumping, and landing that sport places on it.

This is why rehabilitation isn't simply something that happens after surgery. It's part of the treatment itself.

As surgeons, our job isn't just to repair an injury. It's to help patients return to the life they want to live, whether that's playing football, running a marathon, or simply keeping up with their children.

Sometimes, the operation is only the beginning of that journey.

The real success isn't measured by what happens in the operating theatre.
It's measured by the moment a patient returns to the sport they love with confidence, knowing their body is ready for it.

For the last six weeks, getting out of bed in the morning has become an absolute nightmare.

The moment my right heel hits the floor, it feels like a sharp, stabbing knife is piercing the bottom of my foot.

The pain is easily a 9 out of 10, and it forces me to limp and hold onto the walls just to make it to the bathroom.

After about 15 minutes of moving around, the sharp pain finally softens into a dull, throbbing ache, but those first few steps are terrifying.

I’m a primary school teacher, so I’m on my feet for six to seven hours a day. By the end of the school day, my heel feels heavy, bruised, and completely exhausted.

I’m also an avid runner. I usually run about 25 kilometers a week, but recently I switched my routes from soft dirt trails to hard asphalt roads to train for an upcoming 10K.

Strangely enough, the foot doesn’t actually hurt while I’m running. In fact, it warms up nicely and feels fine during the workout.

But about 30 minutes after I stop and sit down to cool off, the pain comes back with a vengeance.
I’ve tried taking over-the-counter painkillers , but it barely touches the pain.

I also tried switching to an older pair of running shoes thinking it might help, but that actually seemed to make it feel worse.

I love running and I can't afford to be limping around my classroom all day I just want to figure out why this is happening and how to fix it.
