សេចក្តីជូនដំណឹង
ស្តីពីការបំផ្លាញឯកសារប្រវត្តិនៃការព្យាបាលរបស់អ្នកជំងឺ
មន្ទីរពេទ្យរ៉ូយ៉ាល់ភ្នំពេញ សូមជូនដំណឹងដល់អ្នកជំងឺ និងសាធារណជនឱ្យបានជ្រាបថា មន្ទីរពេទ្យយើងខ្ញុំនឹងអនុវត្តការបំផ្លាញ ឯកសារប្រវត្តិនៃការព្យាបាលរបស់អ្នកជំងឺ ដែលបានមកទទួលសេវាព្យាបាលនៅមន្ទីរពេទ្យ ក្នុងអំឡុងពេល ឬមុន ឆ្នាំ ២០១៥។
ការបំផ្លាញឯកសារខាងលើ ត្រូវបានអនុវត្តដោយផ្អែកលើ គោលការណ៍ និងរយៈពេលកំណត់ក្នុងការរក្សាទុកឯកសាររបស់មន្ទីរពេទ្យ ដែលមិនលើសពី ១០ ឆ្នាំ។
ប្រសិនបើលោកអ្នកត្រូវការច្បាប់ចម្លងប្រវត្តិនៃការព្យាបាលសូមទាក់ទងមកកាន់ ផ្នែកឯកសារវេជ្ជសាស្ត្រ ដើម្បីដាក់សំណើសុំជាមុន តាមកាលបរិច្ឆេទដែលបានកំណត់ដូចខាងក្រោម៖
កាលបរិច្ឆេទចាប់ផ្តើមស្នើសុំ ពីថ្ងៃទី ៣ ខែកញ្ញា រហូតដល់ថ្ងៃទី ៣ ខែតុលា ឆ្នាំ២០២៦

Destruction of Patient Medical Records
Royal Phnom Penh Hospital would like to inform patients and the public that the hospital will proceed with the destruction of patient medical records for patients who received medical services at the hospital in 2015 and earlier.
The above records will be destroyed in accordance with the hospital's medical record retention policy and the applicable retention period of no more than 10 years.
If you require a copy of your medical records, please contact the Medical Records Department and submit your request before the deadline below:
