Precision matters more than force!
A challenging case involving a 15 mm separated instrument in a maxillary right canine, fractured during primary root canal treatment by a colleague. The tooth had a ceramic veneer that needed to remain completely intact, with an exceptional working length of 29.5 mm, making retrieval significantly more demanding.

Ultrasonic tips were used to carefully expose and loosen the fragment. However, as often happens in these cases, limited coronal access prevented a long instrument from being removed with ultrasonics alone. Without a direct retrieval path, the fragment remained impossible to retrieve safely.

The solution was the Loop Technique, allowing secure engagement and controlled retrieval of the loosened fragment while preserving the veneer and avoiding unnecessary removal of valuable tooth structure.

This case highlights an important principle: ultrasonics are exceptional for troughing around and mobilizing separated instruments, but in restricted access cases, a dedicated grasping technique is often the key to successful retrieval. Conservative endodontics is not only about saving teeth….it is also about preserving existing restorations whenever possible.

Incidental Discovery on LL6 !

A routine radiographic examination revealed an unexpected periapical radiolucency associated with tooth #36, despite the patient being completely asymptomatic.

The initial periapical radiograph identified a separated instrument within the mesial root canal, obstructing access to the apical third and compromising effective disinfection of the root canal system.

Treatment highlights:

• Incidental radiographic detection of an asymptomatic PARL
• Identification of a separated instrument blocking the mesial canal
• Conservative access design with maximum preservation of pericervical dentine, particularly within the coronal third
• Successful retrieval of the fragment using the TFRK loop technique under magnification
• Regaining full canal patency and working length
• Comprehensive chemo-mechanical preparation and irrigation protocol
• Three-dimensional obturation of the root canal system

This case demonstrates how significant apical pathology can remain undetected in the absence of symptoms and highlights the value of routine radiographic examination. It also illustrates that even challenging cases involving separated instruments can often be managed predictably through a conservative nonsurgical approach and should not automatically lead to surgical intervention or extraction when appropriate nonsurgical management can re-establish canal access and allow effective disinfection.
