Apical Microsurgery of Tooth #15

Management of a Buccal Root Perforation Following Previous Root Canal Treatment

A patient presented with a 2-year history of persistent pain on palpation associated with tooth #15 following completion of primary root canal treatment in her home country.

Clinical examination revealed no visible intraoral signs of pathology, with the only finding being localized tenderness to palpation over the buccal aspect of the gingiva adjacent to tooth #15.

Periapical radiographic assessment demonstrated overextension of the obturation material beyond the buccal root canal, raising suspicion of a procedural complication. Subsequent CBCT evaluation confirmed a buccal root perforation located at the curvature of the apical third of the buccal root.

Following diagnosis, apical microsurgery was performed.

The procedure included:

• Surgical access and lesion management
• Removal of the extruded gutta-percha associated with the perforation
• 3 mm apicectomy of the buccal root
• Ultrasonic retrograde cavity preparation using the Acteon® Endo Success Retro Tip AS3D (3 mm straight tip)
• Retrograde sealing with NeoPutty® (Avalon Biomed)

The case highlights the value of CBCT in identifying occult root perforations and demonstrates how modern endodontic microsurgery can successfully manage persistent symptoms associated with iatrogenic complications when conventional retreatment is unlikely to address the cause of failure.
