මිනිස්සු රවටා හොර ජාවාරම් කරන අයට එරෙහිව..

වකුගඩු ගල් ගැන වැරදි තොරතුරුවලට නොරැවටෙමු

පසුගිය දින කිහිපය තුළ Facebook එකේ නැවත නැවතත් share වෙන වීඩියෝවක් මම දැක්කා. එහි පෙන්වන්නේ විශාල ප්‍රමාණයේ මුත්‍රා ගලක් කිසියම් දේශීය ප්‍රතිකාරයක් ලබා ගැනීමෙන් පසු ස්වභාවිකව පිටවූ බවට කරන ප්‍රකාශයක්.

Urological Surgeon කෙනෙකු ලෙස, මෙවැනි වෛද්‍යමය වශයෙන් නොමඟ යවන තොරතුරු සමාජ මාධ්‍ය හරහා පැතිරෙන විට ඒ පිළිබඳව ජනතාව දැනුවත් කිරීම මගේ වගකීමක් ලෙස මම සලකනවා.

අධ්‍යාපනික අරමුණින් එම වීඩියෝවේ screenshot එකක් මෙයට එක් කර තිබෙනවා. එහි පෙන්වන ගලේ ප්‍රමාණය අනුව, සාමාන්‍ය මුත්‍රා මාර්ගය හරහා එවැනි විශාල ගලක් ස්වභාවිකව පිටවීම ඉතාමත් අසම්භාව්‍යයි.

වකුගඩුවේ සිට මුත්‍රා නාලය (ureter) හරහා මුත්‍රාශයට පැමිණ, එවැනි විශාල ගලක් ස්වභාවිකව පිටවීම සාමාන්‍ය වෛද්‍ය විද්‍යාව අනුව අපේක්ෂා කළ නොහැකියි. එවැනි සිදුවීමක් කියා පෙන්වන්නේ නම්, ඉතා දුර්ලභ අසාමාන්‍ය සම්බන්ධතාවයක් (fistula) හෝ වෙනත් පැහැදිලි කිරීමක් පිළිබඳව සලකා බැලිය යුතුයි.

වීඩියෝවේ පෙන්වන කුඩා ගල් කැබලි නම් වෙනම කාරණයක්. කුඩා මුත්‍රා ගල් ස්වභාවිකව පිටවීම සාමාන්‍යයෙන් සිදුවිය හැකි දෙයක්. ඒ සඳහා නවීන Urology තුළ හොඳ විද්‍යාත්මක සාක්ෂි තිබෙනවා.

ඒ නිසා කුඩා ගල් කිහිපයක් ප්‍රතිකාරයකින් පසුව පිටවීම පමණක් පෙන්වා, එම ප්‍රතිකාරයෙන් විශාල වකුගඩු ගල්ද පිට කළ හැකි බවට කරන ප්‍රකාශයක් විශ්වාස කිරීම නිවැරදි නැහැ.

වකුගඩු ගල් බහුලව දක්නට ලැබෙන ප්‍රදේශවල, ජනතාවගේ බිය සහ අසරණකම ප්‍රයෝජනයට ගනිමින් මෙවැනි ප්‍රකාශ කරන පුද්ගලයන් සිටීම කණගාටුවට කරුණක්.

කරුණාකර මේ කරුණ මතක තබාගන්න:

මගේ එකම අරමුණ වන්නේ Facebook වීඩියෝවක් නිසා නිසි වෛද්‍ය ප්‍රතිකාර ප්‍රමාද නොකරන ලෙස ජනතාව දැනුවත් කිරීමයි.

ඔබට හෝ ඔබේ පවුලේ කෙනෙකුට වකුගඩු ගලක් හෝ මුත්‍රා නාලයේ ගලක් තිබේ නම්, නිවැරදි imaging පරීක්ෂණයක් කර සුදුසුකම් ලත් Urologist කෙනෙකුගෙන් උපදෙස් ලබාගන්න.

ගලේ ප්‍රමාණය, ස්ථානය සහ මුත්‍රා මාර්ගයට ඇති අවහිරතාව අනුව ප්‍රතිකාරය තීරණය කළ යුතුයි.

Kidney Stones: Please Don’t Be Misled

Recently, I noticed a video circulating repeatedly on Facebook claiming that a very large urinary stone was passed spontaneously after treatment by a traditional practitioner.

As a young urological surgeon, I feel that we have a responsibility to respond when medical misinformation is widely shared—especially when it may influence patients to delay proper treatment.

I have attached a screenshot from the video for educational purposes. The stone shown appears to be far too large to expect spontaneous passage through the normal urinary tract.

From a urological and anatomical perspective, a stone of that size cannot normally travel from the kidney through the ureter and pass naturally. Passage of a stone of that size would be extraordinarily unlikely and would raise the possibility of an unusual abnormal communication (fistula) or another explanation. Such situations are extremely rare.

The small stone-like fragments shown in the video are a completely different matter. Small urinary stones can indeed pass spontaneously, and this is well established in modern urology. Therefore, seeing small stones pass after taking a particular medicine or herbal preparation does not prove that the treatment dissolved or removed a large kidney stone.

This is particularly important in areas where urinary stone disease is common. Unfortunately, some people may take advantage of patients’ fears and financial difficulties by making claims that are not supported by medical evidence.

Please remember:

A large kidney stone should not simply be left untreated based on claims that it will eventually pass on its own.

A stone that causes persistent obstruction can damage the kidney over time. If obstruction is severe or associated with infection, it can become a medical emergency. Delayed treatment can result in irreversible loss of kidney function and, in severe cases, life-threatening complications.

I am not writing this post to attack any individual or traditional practice. My intention is simple:

Please do not let a Facebook video replace proper medical assessment.

If you or someone you know has a kidney or ureteric stone, please get appropriate imaging and consult a qualified urologist to determine the stone’s size, location, degree of obstruction, and the safest treatment.

Small stones may pass naturally.
Large stones generally require proper urological assessment and, depending on their size and location, may require procedures such as SWL, ureteroscopy, or PCNL.

Your kidney is not something to gamble with.

Please share this information if you think it may prevent someone from delaying necessary treatment.

| சிறுநீரகக் கற்கள் பற்றிய தவறான தகவல்களை நம்ப வேண்டாம்

சமீபத்தில் Facebook-இல் மீண்டும் மீண்டும் பகிரப்பட்ட ஒரு காணொளியை நான் கவனித்தேன். அதில் மிகப் பெரிய சிறுநீரக/சிறுநீரகப் பாதைக் கல் ஒன்று, ஒரு குறிப்பிட்ட பாரம்பரிய சிகிச்சைக்குப் பிறகு இயற்கையாக வெளியேறியதாகக் கூறப்படுகிறது.

ஒரு இளம் Urological Surgeon என்ற முறையில், மருத்துவ ரீதியாக தவறாக வழிநடத்தக்கூடிய தகவல்கள் சமூக ஊடகங்களில் பரவும்போது அதற்கு பதிலளித்து மக்களுக்கு சரியான தகவலை வழங்குவது நமது பொறுப்பு என்று நான் கருதுகிறேன்.

கல்வி நோக்கத்திற்காக அந்த காணொளியில் இருந்து ஒரு screenshot-ஐ இணைத்துள்ளேன். அதில் காட்டப்படும் கல்லின் அளவைப் பார்க்கும்போது, அத்தகைய பெரிய கல் சாதாரண சிறுநீரகப் பாதையின் வழியாக இயற்கையாக வெளியேறுவது மிகவும் சாத்தியமற்றது.

சிறுநீரகத்திலிருந்து ureter வழியாக சிறுநீர்ப்பைக்கு வந்து, அத்தகைய பெரிய கல் இயற்கையாக வெளியேறுவது சாதாரண உடற்கூறியல் மற்றும் Urology அறிவின் அடிப்படையில் எதிர்பார்க்க முடியாத ஒன்று. அவ்வாறு நிகழ்ந்ததாகக் கூறப்பட்டால், மிகவும் அரிதான fistula போன்ற அசாதாரண தொடர்பு அல்லது வேறு மருத்துவ விளக்கம் இருக்கிறதா என்பதைப் பரிசீலிக்க வேண்டும்.

காணொளியில் காட்டப்படும் சிறிய கல் துண்டுகள் வேறு ஒரு விஷயம். சிறிய சிறுநீரகக் கற்கள் சில சமயங்களில் எந்த அறுவை சிகிச்சையும் இல்லாமல் தானாகவே வெளியேறலாம். இது நவீன Urology-யில் நன்கு அறியப்பட்ட உண்மை.

எனவே, சிறிய கற்கள் வெளியேறியதை மட்டும் காட்டி, அதே சிகிச்சையால் மிகப் பெரிய சிறுநீரகக் கற்களும் தானாக வெளியேறும் என்று கூறுவது மருத்துவ ரீதியாக சரியான முடிவு அல்ல.

சிறுநீரகக் கற்கள் அதிகமாக காணப்படும் பகுதிகளில், மக்களின் பயத்தையும் பொருளாதார சிரமங்களையும் பயன்படுத்தி இப்படியான மருத்துவ ஆதாரமற்ற விளம்பரங்கள் செய்யப்படுவது மிகவும் கவலைக்குரியது.

தயவுசெய்து இந்த முக்கியமான விடயத்தை நினைவில் வைத்துக்கொள்ளுங்கள்:

பெரிய சிறுநீரகக் கல் “எப்படியாவது தானாக வெளியேறிவிடும்” என்று நினைத்து உரிய சிகிச்சையை தாமதிப்பது ஆபத்தானதாக இருக்கலாம்.

ஒரு கல் காரணமாக சிறுநீர் செல்லும் பாதை நீண்ட காலம் அடைபட்டிருந்தால், அந்த சிறுநீரகத்திற்கு நிரந்தரமான சேதம் ஏற்படலாம். அதனுடன் infection சேர்ந்தால் அது ஒரு அவசர மருத்துவ நிலையாக மாறக்கூடும்.

சிகிச்சையை தாமதிப்பதால் நிரந்தர சிறுநீரக செயலிழப்பு ஏற்படலாம்; கடுமையான சந்தர்ப்பங்களில் உயிருக்கும் ஆபத்து ஏற்படலாம்.

எந்த ஒரு நபரையும் அல்லது பாரம்பரிய மருத்துவ முறையையும் தனிப்பட்ட முறையில் தாக்குவது எனது நோக்கம் அல்ல.

எனது ஒரே நோக்கம் — Facebook காணொளி ஒன்றை நம்பி, தேவையான மருத்துவ சிகிச்சையை யாரும் தாமதிக்கக்கூடாது என்பதே.

உங்களுக்கோ அல்லது உங்கள் குடும்பத்தில் ஒருவருக்கோ சிறுநீரகக் கல் அல்லது ureteric stone இருந்தால், சரியான imaging பரிசோதனை செய்து, தகுதியான Urologist ஒருவரை அணுகுங்கள்.

கல்லின் அளவு, இருப்பிடம் மற்றும் urinary obstruction ஆகியவற்றைப் பொறுத்து சரியான சிகிச்சை தீர்மானிக்கப்பட வேண்டும்.

உங்கள் சிறுநீரகம் மிகவும் விலைமதிப்பற்றது. அதை ஆபத்தில் விடாதீர்கள்.

இந்த தகவல் ஒருவருக்காவது பயனுள்ளதாக இருக்கும் என்று நினைத்தால் தயவுசெய்து பகிருங்கள்.

Lets learn some interesting Urology today,

My First Interesting patient After Completing My Overseas Training

This was the first interesting case I managed after returning from my overseas training.

A young man came to our clinic with a forgotten ureteric stent.

He had undergone surgery during childhood for a congenital blockage between the kidney and the ureter. Since then, he required regular ureteric stent exchanges. His last stent was changed in 2022.

Unfortunately, he became anxious about the next stent change and did not return for follow-up until July 2026. By then, the stent had remained inside for almost four years.

Before treatment, we performed an X-ray to assess the position and condition of the stent and to look for common complications of long-term stents, such as stone formation and encrustation.

The X-ray showed that the stent had broken at the level of the pelvic brim. Fortunately, there were no visible stones, stent migration or infection.

We proceeded with cystoscopy and ureteroscopy. During cystoscopy, there were no stones around the lower end of the stent, and the broken lower fragment was removed successfully. Ureteroscopy up to the renal pelvis showed no stones, no encrustation, and no obvious recurrent PUJ obstruction. The remaining part of the stent was removed easily, so a new stent was not inserted.

The patient was discharged with plans for a CT IVU and a DTPA renal scan to assess kidney drainage and decide on further management. If he develops pain, fever, or urinary obstruction before then, urgent stenting will be performed.

Take-home message

A ureteric stent should never be forgotten. It should always be removed or exchanged at the recommended interval. In most routine situations, a stent should be changed within 6 months. The sooner it is removed or exchanged, the safer it is.

It is a true privilege and an honour to care for the people of Sri Lanka, especially those living in rural communities.

Every patient reminds us of the importance of patient education, regular follow-up, and timely access to healthcare. I remain committed to doing my best to improve public awareness so that preventable situations like this become less common in the future.

යූරෝලොජි දිනපොතින් පිටුවක්…

විදේශ පුහුණුව අවසන් කර පැමිණි පසු මගේ පළමු විශේෂ රෝගියා

තරුණ රෝගියෙකු අමතක වූ යූරෙටරික් ස්ටෙන්ට් (Forgotten ureteric stent) එකක් සමඟ අප වෙත පැමිණියා.

ඔහුට කුඩා කාලයේදී වකුගඩුව සහ මුත්‍රා නාලය අතර උපතින් තිබූ අවහිරතාවයක් (PUJ obstruction) සඳහා ශල්‍යකර්මයක් සිදු කර තිබුණි. ඉන් පසු නිතිපතා යූරෙටරික් ස්ටෙන්ට් මාරු කර ගැනීම අවශ්‍ය විය. අවසන් වරට ස්ටෙන්ට් එක මාරු කර තිබුණේ 2022දීය.

අවාසනාවකට, ඊළඟ ස්ටෙන්ට් මාරු කිරීම පිළිබඳ ඇති වූ බිය නිසා ඔහු නැවත සායනයට පැමිණියේ 2026 ජූලි මාසයේදීය. ඒ වන විට ස්ටෙන්ට් එක වසර හතරකට ආසන්න කාලයක් ශරීරය තුළ තිබී ඇත.

ප්‍රතිකාරයට පෙර X-ray පරීක්ෂණයක් සිදු කළේ ස්ටෙන්ට් එකේ ස්ථානය, තත්ත්වය සහ දිගු කාලයක් තිබීම නිසා ඇතිවිය හැකි ගල් හෝ ලුණු තැන්පතු (encrustation) තිබේදැයි පරීක්ෂා කිරීම සඳහාය.

X-ray පරීක්ෂණයෙන් ස්ටෙන්ට් එක ශ්‍රෝණි ප්‍රදේශයේදී කැඩී තිබූ බව පෙනුණත්, වාසනාවකට මෙන් ගල් කිසිවක් නොපෙනුණි.

ඉන්පසු cystoscopy සහ ureteroscopy සිදු කරන ලදී. ස්ටෙන්ට් එකේ පහළ කොටස වටා ගල් නොතිබූ අතර, කැඩී තිබූ පහළ කොටස අපහසුවකින් තොරව ඉවත් කළෙමු. පසුව වකුගඩුව දක්වා ureteroscopy කැමරාව කිරීමේදී ගල් හෝ ලුණු තැන්පතු කිසිවක් නොතිබූ අතර නැවත PUJ අවහිරතාවයක්ද නොපෙනුණි. ඉතිරි වූ ස්ටෙන්ට් කොටස පහසුවෙන් ඉවත් කළ අතර නව ස්ටෙන්ට් එකක් නොදැමීමට තීරණයක් ගතිමි.

රෝගියා CT IVU සහ DTPA වකුගඩු පරීක්ෂණ සඳහා යොමු කරන ලදී. එම ප්‍රතිඵල අනුව ඉදිරි ප්‍රතිකාර සැලසුම් කරනු ඇත. මේ අතර වේදනාව, උණ හෝ මුත්‍රා අවහිර වීමක් ඇති වුවහොත් වහාම නැවත ස්ටෙන්ට් එකක් දමනු ඇත.

මතක තබා ගන්න

යූරෙටරික් ස්ටෙන්ට් එකක් අමතක කර තැබිය නොහැක. වෛද්‍යවරයා නියම කරන කාලයට පෙර එය ඉවත් කිරීම හෝ මාරු කිරීම අත්‍යවශ්‍යය. සාමාන්‍යයෙන් මාස 6ක් ඇතුළත මාරු කිරීම සුදුසු වන අතර, ඉක්මනින් කිරීම වඩාත් ආරක්ෂිතය.

විශේෂයෙන්ම ශ්‍රී ලංකාවේ ග්‍රාමීය ප්‍රදේශවල ජනතාවට ප්‍රතිකාර කිරීමට ලැබීම මා ලැබූ විශාල ගෞරවයක් සහ වරප්‍රසාදයක් ලෙස සලකමි.

සෑම රෝගියෙකුම ජනතාව දැනුවත් කිරීමේ, නියමිත කාලයට වෛද්‍ය පරීක්ෂණ සඳහා පැමිණීමේ සහ සෞඛ්‍ය සේවාව සමඟ අඛණ්ඩව සම්බන්ධ වී සිටීමේ වැදගත්කම අපට නැවතත් මතක් කර දෙයි. මෙවැනි වළක්වා ගත හැකි තත්ත්වයන් අනාගතයේදී අවම කර ගැනීම සඳහා මහජන දැනුවත් කිරීම සඳහා මගේ උපරිම දායකත්වය ලබා දීමට මම කැපවී සිටිමි.

රෝගීන්, පවුල් සහ සෞඛ්‍ය වෘත්තිකයන් එක්ව කටයුතු කළහොත් දැනුවත් කිරීම, නියමිත පසු විපරම් සහ කාලෝචිත ප්‍රතිකාර මඟින් වඩා හොඳ ප්‍රතිඵල ලබා ගත හැකිය.

வெளிநாட்டு பயிற்சியை முடித்து பணிக்கு திரும்பிய பிறகு நான் சிகிச்சையளித்த முதல் சுவாரஸ்யமான நோயாளர்

வெளிநாட்டு சிறுநீரக மற்றும் சிறுநீரக அறுவைச் சிகிச்சை (Urology) பயிற்சியை முடித்து பணிக்கு திரும்பிய பிறகு நான் சந்தித்த முதல் சுவாரஸ்யமான நோயாளர் இவர்.

இளம் வயதுடைய ஒருவர் மறக்கப்பட்ட யூரெடெரிக் ஸ்டென்ட் (Forgotten ureteric stent) உடன் எங்களிடம் வந்தார்.

சிறுவயதில், சிறுநீரகத்திற்கும் சிறுநீர்க்குழாயிற்கும் இடையிலான பிறவிக்குறைபாடு (PUJ obstruction) காரணமாக அவருக்கு அறுவை சிகிச்சை செய்யப்பட்டிருந்தது. அதன் பின்னர், யூரெடெரிக் ஸ்டென்டை குறிப்பிட்ட கால இடைவெளியில் மாற்றி வர வேண்டியிருந்தது. கடைசியாக அது 2022 ஆம் ஆண்டு மாற்றப்பட்டது.

துரதிர்ஷ்டவசமாக, அடுத்த ஸ்டென்ட் மாற்றும் சிகிச்சையைப் பற்றிய பயம் காரணமாக அவர் மீண்டும் மருத்துவமனைக்கு வரவில்லை. இறுதியாக 2026 ஜூலை மாதத்தில் எங்களைச் சந்தித்தார். அப்போது ஸ்டென்ட் கிட்டத்தட்ட நான்கு ஆண்டுகளாக உடலுக்குள் இருந்தது.

சிகிச்சைக்கு முன், ஸ்டென்டின் நிலை, அதன் நிலைத்தன்மை மற்றும் நீண்ட காலம் இருந்ததால் கல் அல்லது உப்புப் படிவு (encrustation) ஏற்பட்டுள்ளதா என்பதை மதிப்பிட எக்ஸ்-ரே எடுக்கப்பட்டது.

எக்ஸ்-ரேவில், ஸ்டென்ட் இடுப்பு பகுதியில் உடைந்திருந்தது. அதிர்ஷ்டவசமாக, கற்கள் எதுவும் காணப்படவில்லை.

அதனைத் தொடர்ந்து cystoscopy மற்றும் ureteroscopy செய்யப்பட்டது. ஸ்டென்டின் கீழ்பகுதியில் கற்கள் எதுவும் இல்லை. உடைந்திருந்த கீழ்பகுதி வெற்றிகரமாக அகற்றப்பட்டது. பின்னர் சிறுநீரகம் வரை ureteroscopy செய்து பார்த்தபோது கற்களோ, உப்புப் படிவமோ இல்லை. மீண்டும் PUJ அடைப்பு இருப்பதற்கான அறிகுறிகளும் காணப்படவில்லை. மீதமிருந்த ஸ்டென்டும் எளிதாக அகற்றப்பட்டதால் புதிய ஸ்டென்ட் வைக்கப்படவில்லை.

நோயாளிக்கு CT IVU மற்றும் DTPA சிறுநீரக பரிசோதனைகள் திட்டமிடப்பட்டுள்ளன. அவற்றின் முடிவுகளின் அடிப்படையில் அடுத்த சிகிச்சை தீர்மானிக்கப்படும். இதற்கிடையில் வலி, காய்ச்சல் அல்லது சிறுநீர் அடைப்பு ஏற்பட்டால் உடனடியாக புதிய ஸ்டென்ட் வைக்கப்படும்.

நினைவில் கொள்ள வேண்டிய முக்கிய செய்தி

யூரெடெரிக் ஸ்டென்டை ஒருபோதும் மறந்து விடக்கூடாது. மருத்துவர் கூறிய காலத்திற்குள் அதை அகற்றவோ அல்லது மாற்றவோ வேண்டும். பொதுவாக 3–6 மாதங்களுக்குள் மாற்றுவது பாதுகாப்பானது. விரைவாக மாற்றுவது இன்னும் சிறந்தது

குறிப்பாக இலங்கையின் கிராமப்புற மக்களுக்கு மருத்துவ சேவை வழங்கும் வாய்ப்பு கிடைத்திருப்பது எனக்கு மிகுந்த பெருமையும் மரியாதையும் அளிக்கிறது.
