A Class III? Direct composite, every time ❤️

Closing small facets on the incisal edge without increasing tooth length? Also direct 🫶🏻

No need to overcomplicate what can be done in a simple, predictable, and minimally invasive way without covering the enamel with resin 🙏

This case was restored using the Modified Mylar Pull Technique (MMPT) for the Class III and Connect the Dots principles to rebuild the incisal facets 👏

Effective. Efficient. Predictable. That’s the kind of dentistry I enjoy most ❤️❤️❤️

I love predictable dentistry. Constantly chipping composites are just not my thing.

That’s why I’m quite obsessive about overbite and overjet. Unlike many Instagram cases, I honestly don’t see many patients where I can simply add 2+ mm to the maxillary anteriors without changing the occlusal vertical dimension.

But every few years, one of these patients shows up.

In this case, the vertical maxillary deficiency meant the incisors were not visible - even when smiling. It was one of those rare cases where all six maxillary anteriors needed a significant increase in length.

Welcome to the cutback.

I first restored the full length and width with a milky-white enamel shade composite. Then I removed the rubber dam, checked the proportions, smile, and overall shapes. Happy with everything? Dam back on, cutback, layering (no tints), finishing, and polishing.

The cutback, layering, finishing, and polishing for all six teeth took around 1–1.5 hours. I added length on premolars a bit as well.

As always, my goal isn’t to create a beautiful result just for day one. It’s to create restorations that survive function. Because predictable dentistry will always beat pretty dentistry that keeps chipping.

I am just doomed - my patients are amazing, but so many are either dentists or family of dentists, dental technicians, hygienists, etc.

Some let me do what I do. And some … are fun 😀
This was the first time I suggested that I close diastemas before prepping for new veneers (so we have better emergence profile palatally) 🤞🏻
As you can see, my colleague couldn’t just sit around, so I made him work a bit (of course, I choose non painful and fun composite restos and removing very well adapted and cemented temporary - he can do this himself) 😀

We have a one year recall here 🫶🏻

Aligner softwares can’t detect restorations and they align teeth with composite as if they were genuine teeth. This is why for patients that live further from Vilnius I can’t control the last steps before restorations. Sometimes we just don’t have space just because biomechanics don’t allow it.

Life is not perfect.

So I sacrificed buccal embrasure. I can’t show you a perfect single tooth resto, but smile is beautiful just like my patient 🫶🏻
