Šodien (13.05.2026.) piedalījos Dr. Marylene Cloitre vadītajā vebinārā “Kompleksi pēctraumas stresa traucējumi (KPTST): inovācijas konceptualizācijā, novērtēšanā un ārstēšanā”, ko organizēja Eiropas Traumu un disociācijas biedrība (European Society for Trauma and Dissociation - ESTD). Tajā tika uzsvērta ilgi gaidītā pāreja no jautājuma “vai KPTST pastāv?” uz “kā mēs to varam precīzāk diagnosticēt un ārstēt?”, jo KPTST šobrīd ir atzīta diagnoze Pasaules veselības organizācijas (World Health Organization (WHO)) SSK-11 klasifikatorā, nevis tikai teorētisku diskusiju objekts. Paldies par rūpīgu tēmas pārskatu! ❤️

Svarīgs atgādinājums: darbā ar traumām jāstrādā ne tikai ar bailēs balstītiem simptomiem, bet arī ar identitātes traucējumiem, emociju regulācijas grūtībām un attiecību problēmām, jo KPTST terapija bieži prasa vairāk nekā tikai traumatisko atmiņu apstrādi. Esošās pēctraumas stresa traucējumu (PTST) terapijas pieejas joprojām darbojas, taču tās mēdz būt mazāk efektīvas cilvēkiem ar traumatiskām pieredzēm bērnībā. Turklāt speciālistiem būtu jāizvērtē disociācijas simptomi, nevis jāpieņem, ka visiem cilvēkiem KPTST izpaužas vienādi.

PTST (atkārtota traumas pārdzīvošana (re-experiencing), izvairīšanās, pastāvīga apdraudējuma izjūta) ≠ KPTSD (atkārtota traumas pārdzīvošana (re-experiencing), izvairīšanās, pastāvīga apdraudējuma izjūta + emociju regulācijas grūtības, negatīvs priekšstats par sevi, attiecību problēmas [+ disociācija])
- ja cilvēkam ir KPTST, papildu PTST diagnoze nav loģiska

Robežstāvokļa personības traucējumi (PTST simptomi nav daļa no šīs diagnozes; mainīgs, nestabils priekšstats par sevi; nestabilas attiecības; bailes no pamešanas; augsts suicidālitātes un nesuicidāla paškaitējuma risks) ≠ KPTST (PTST simptomi ir obligāti; konsekventi negatīvs priekšstats par sevi; izvairīšanās attiecībās; zemāks suicidālitātes un nesuicidāla paškaitējuma risks)
- dažiem cilvēkiem var atbilst gan robežstāvokļa personības traucējumi, gan KPTST, it īpaši, ja pieredzē bijušas smagas attīstības traumas

Today (May 13, 2026), I participated in Dr. Marylene Cloitre’s webinar, “Complex PTSD: Innovations in Conceptualization, Assessment, and Treatment,” organized by European Society for Trauma and Dissociation. It emphasized the long-awaited transition from “Does CPTSD exist?” to “How do we assess and treat it more precisely?”, as CPTSD is now an established diagnosis (in World Health Organization ICD-11) rather than a theoretical debate. Thank you for a thorough and insightful overview of the topic! ❤️

A good reminder: trauma treatment should address both fear-based symptoms AND disturbances in identity, emotional regulation, and relationships, as CPTSD treatment often needs to go beyond trauma memory processing. Existing PTSD treatments still work, however, they tend to be less effective for childhood trauma survivors. In addition, clinicians should screen for dissociation rather than assuming all individuals with CPTSD present in the same way.

PTSD (re-experiencing, avoidance, sense of threat) ≠ CPTSD (re-experiencing, avoidance, sense of threat + affect dysregulation, negative self-concept, disturbed relationships [+ dissociation])
- If a person has CPTSD, an additional PTSD diagnosis does not make sense.

BPD (PTSD symptoms are not part of the diagnosis; alternating, unstable self-concept; unstable relationships; fear of abandonment; high suicidality and non-suicidal self-injury) ≠ CPTSD (PTSD symptoms required; consistently negative self-concept; relational avoidance; lower suicidality and non-suicidal self-injury)
- Some individuals may meet criteria for both BPD and CPTSD, particularly when severe developmental trauma is involved.

Yesterday (May 6, 2025), a few members of EMDR Latvia, along with members of other new and emerging national EMDR associations, took the opportunity to attend the workshop “Working Towards Accreditation” led by the experienced Maeve Crowley. The information shared will help EMDR Latvia encourage and motivate its members to become accredited, while also helping them prepare for accreditation more efficiently. ✔️

Lisa Feldman Barrett apstrīd grāmatā "Ķermenis atceras" pausto ideju ("The body keeps the score", Bessel van der Kolk), ka trauma galvenokārt glabājas ķermenī. Viņa saka, ka smadzenes (te, iespējams, varētu runāt par visu autonomo nervu sistēmu) ir tās, kas rada visas pieredzes, arī traumatiskās. Svarīgi saprast, ka smadzenes nav tikai reaģējošas - tās prognozē, izmantojot līdzšinējās dzīves pieredzes un tādējādi taupot enerģiju. Turklāt smadzeņu galvenais uzdevums ir regulēt ķermeņa darbību, piemēram, sirdsdarbību, elpošanu, hormonu darbību, enerģiju. Atveseļošanās no traumas notiek, kad smadzenes iemācās jaunas prognozes. Tam ir nepieciešamas jaunas pieredzes drošā vidē. 🩵

EMDR terapija ir viena no psihoterapijas pieejām, kas palīdz atjaunināt smadzeņu prognozes.

Lisa Feldman Barrett challenges the idea presented in The Body Keeps the Score by Bessel van der Kolk that trauma is primarily stored in the body. She argues that the brain (and arguably, this could be understood more broadly as the entire autonomic nervous system) is what creates all experiences - including traumatic ones. It’s important to understand that the brain is not merely reactive - it predicts. It uses past life experiences to anticipate what might happen next, which helps conserve energy. Moreover, one of the brain’s main functions is regulating the body’s processes, such as heart rate, breathing, hormonal activity, and energy use. Recovery from trauma happens when the brain learns new predictions. That requires new experiences in a safe environment. 🩵

EMDR therapy is one psychotherapy approach that helps update the brain’s predictions.

Dažas (no daudzām) atziņām:

- ir jāatmet vēlme pielāgot cilvēku savai iecienītajai psihoterapijas pieejai, un tā vietā par prioritāti jāizvirza viņa unikālās dzīves vēstures izpēte
- stabilizācija nav tikai atbalsts darbam ar traumatiskām atmiņām - daudziem pamata attīstības spēju veidošana ir lielākā darba daļa
- veiksmīgai stabilizācijai nepieciešamas četras būtiskas prasmes: drošība, emociju regulācija, mentalizācija un klātesamība
- daudzi cilvēki nepanes iekšup vērstu apzinātību - viņiem bieži vien ir jāsāk ar apkārtējās pasaules, nevis iekšējo sajūtu apzināšanos
- ietver atvērtu hipotēžu uzturēšanu par apkārtējo nodomiem, pamatojoties uz neverbālām norādēm un attiecību modeļiem, nevis pieņēmumu, ka cilvēks precīzi zina, ko citi domā
- emociju regulācijas mērķis nav atbrīvoties no nepatīkamām sajūtām, bet gan pieņemt un piedzīvot tās kā pārejošus "iekšējos laikapstākļus"
- emociju regulācijai var palīdzēt uzmanības svārstīšana (pendulation) starp nepatīkamām un patīkamām vai neitrālām sajūtām
- ja pastiprinās paškaitējums, suicidālitāte vai atmiņu uzplaiksnījumi (flashbacks), temps ir pārāk straujš, pat ja cilvēks apgalvo pretējo
- terapija jāsāk ar pieaugušā "Es" stiprināšanu un sadarbības veidošanu starp disociatīvām daļām (ja tādas ir; nejaukt ar ego daļām (ego states)), izvairoties no pāragra darba ar bērna daļām (ja tādas ir) bez pieaugušā "Es" klātbūtnes

Yesterday (April 30, 2026), I participated in Kathy Steele's clinical masterclass "Stabilisation, Pacing and Clinical Judgement in Complex Trauma" organized by MIND in MIND. Thank you for this invaluable opportunity! ❤️

Some (of many) insights:

- we must let go of the desire to fit a person into our preferred psychotherapeutic approach and instead prioritize the exploration of their unique life history
- stabilization is not merely a support for working with traumatic memories - for many, building basic developmental capacities is the major part of the work
- successful stabilization requires four essential skills: safety, emotion regulation, mentalization, and being present
- many people cannot tolerate inward-focused mindfulness - they often need to start with awareness of the surrounding world rather than internal sensations
- mentalization involves maintaining open hypotheses about the intentions of others, based on non-verbal cues and relationship patterns, rather than the assumption that one knows exactly what others are thinking
- the goal of emotion regulation is not to get rid of unpleasant ones, but to accept and experience them as passing "internal weather"
- shifting attention (pendulation) between unpleasant and pleasant or neutral sensations can help with emotion regulation
- if self-harm, suicidality, or flashbacks intensify, the pace is too fast, even if the person claims otherwise
- therapy should begin with strengthening the adult self and building cooperation between dissociative parts (if present; not to be confused with ego states), avoiding premature work with child parts without the presence of the adult self

Today (April 23, 2026), I found myself in awe (and a bit starstruck) attending the live Legacy Interview with Kathy Steele by MINDinMIND Interviews. When I listen to Kathy Steele, Dolores Mosquera, Ana M. Gomez (AGATE Institute:Ana Gomez Attachment Trauma Education), and Pat Ogden, I feel curiosity, humility and hope to tolerate the untolerable, which is present in the process for those of us who do trauma work. Thank you for the depth of humanity put into words today! ❤️

Today (April 22, 2026), I gave a lecture on “The Importance of Healthy and After a Long Day at the Office.” 💤

When I received the invitation from TestDevLab, I didn’t yet realize how interesting and rewarding I would find preparing and delivering this presentation. I also couldn’t resist mentioning the Window of Tolerance, the Predictive Brain model, and the Theory of Constructed Emotion while still staying on topic. In the end, I think it all came together quite well.
