Why do changes in serum electrolytes cause changes in P wave, QT interval, T wave and U wave❓
⚠️The 5 Phases of Cardiac Action Potential (Phase 0-4)
(Rapid upstroke)
⚠️Correlating Action Potential with ECG
⚠️Integrating Electrolytes, Action Potential, and ECG Changes
🧪 Potassium (K+) - The "Repolarization" Ion
• Hyperkalemia: High extracellular K+ increases the activity of outward K+ channels . This causes a steep Phase 3, resulting in tall, "tented" T waves.
• The Paradox: As K+ rises, it depolarizes the Resting Membrane Potential (RMP). A less negative RMP prevents Voltage-Gated Sodium (Na+) Channels from resetting (they stay in the INACTIVATED state).
• ECG Impact: Slowed conduction causes Widened QRS and eventually a loss of P waves.
(So hyperkalemia causes small P, Tall T, widened QRS)
🧪Calcium (Ca2+) | The "Plateau" Ion
• Calcium primarily affects Phase 2 (The Plateau).
• Hypocalcemia: Extends the time Ca2+ enters the cell. This lengthens Phase 2, lengthening the ST segment (ပုံမှန်ဆို outer cell membrane relatively negative compared to inner cell, Calcium က positively charged ဖြစ်တာကြောင့် outer cell membrane ကို bind လုပ်ထားပါတယ်။ Hypocalcemia မှာ outer cell membrane ကို bind မလုပ်နိုင်၊ voltage gated L type Calcium channel တွေ အကြာကြီးပွင့်၊ plateau phase ကြီးပိုကြာ၊ ST segment ပိုရှည်)
• ECG Impact: Prolonged QT interval (specifically a long ST segment).
• Hypercalcemia: Shortens Phase 2.
• ECG Impact: Shortened QT interval.
🧪Magnesium (Mg2+) | The "Stabalizer"
• Magnesium stabilizes the membrane and helps facilitate the outward flow of K+ during Phase 3.
• Hypomagnesemia: Because magnesium depletion often leads to secondary hypokalemia and hypocalcemia, the ECG findings are often a "mixed bag".
ECG impact:
• Prolonged QT Interval: Primarily due to associated hypocalcemia and the delayed repolarization of the ventricles.
• Prominent U waves: Often seen, reflecting the underlying hypokalemia.
• Increased Arrhythmogenicity: This is the most critical clinical aspect. Hypomagnesemia makes the myocardium hyper-excitable, predisposing the heart to Torsades de Pointes (a form of polymorphic ventricular tachycardia).
• Hypermagnesemia: This is rare and usually occurs in cases of renal failure or excessive intake. It acts as a calcium antagonist.
ECG impact:
• Broadened QRS Complex: High levels of magnesium block calcium channels, which slows down the conduction velocity, similar to hyperkalemia.
• PR Prolongation: Slowed AV nodal conduction.
• AV Block: Severe cases can lead to complete heart block and, eventually, cardiac arrest.
