A characteristic MRI appearance where the midbrain looks like a “molar tooth” due to abnormal brainstem and cerebellar development.

Structures involved:
Thickened & horizontally oriented superior cerebellar peduncles
Deep interpeduncular fossa
Hypoplasia/aplasia of cerebellar vermis

Most likely diagnosis:
