✅ WHO CAN APPLY?
Social Aid beneficiaries with a monthly household income not exceeding Rs 40,000.
👁️ HOW TO APPLY?
1️⃣ Book an eye test with us
Get your eyeglass prescription and spectacles quotation from MOON Optical.
2️⃣ Visit your nearest Social Security Office
Submit your prescription and quotation together with the required documents*, including originals and photocopies, where applicable:
📌 National Identity Card of applicant / proxy / spouse
📌 Birth Certificate of applicant / spouse
📌 Marriage Certificate
📌 Payslips of working household members, if any
📌 Documentary evidence of income from other sources
